11/13 Public Health Meeting
-Applications for students. Send to all of the listserves. Send out this Friday, due date Dec 16th. Let know decisions will be made by first week back. -$600 due first Friday after winter break back. Case-by-case basis if cannot pay by then ("please let us know if you will not be able...") -4th years: Rachel Baskin.

-New IRB for CHW project must be filled out.
-Jess and Olga Nov trip:
-meeting with Dr. Water Encarnacion, Dra. Perez. "Padrinos" with partnership between us and DOH. Expand Los Botaos clinic, add ER, add bathroom, and another Doctor's office. Met with engineer in charge of projects, we are getting architectural plans. Will be brought up at board. Our name and logo will currently go on. Would help us with customs, we could use the clinic. We would be able to work with community more. The problem is financing. Ultimately the goal would be us to use this as a master clinic so we can go down with pre-triaged patients and treat the most urgent and important/specialized cases.
-DOH program essentially does nothing except for fichas
-IRB signed
-Contract for Doctora - internet "spotty" and didn't ever get contract. Her computers we gave her apparently don't work...
-need to save on external HD
-we set up a gmail account that we can monitor how often she checks. She had 5000 freaking unread emails! She has not contacted us... -Doctora's perspective: Arelis is the main one working, all the townspeople go to her. HTN and DM are 2 of main problems as well as sickle cell disease and asthmatics. Pregnant women are not receiving prenatal care. Why has she not told us this?! She didn't think Franklin was doing much except read to CHW's, (but CHW's felt he was awesome!). She doesnt seem to understand the role of the CHW project. Glucometer not working, running out of strips. Documentation? She does not seem to have the list of chronic patients and says it is "locked" in her office...
-add to patient satisfaction: assess what they think of doctora, do they visit her? How often? -Met with CHW's: they are doing things as layed out in Donde no Hay Doctor such as making ORT for diarrhea, but this has not been communicated to Dra. Documentation? Should be filling out back of sheet in DNHD. Explained HTN project and that roles would be split (not all Arelis). The key is that the book we gave has been a great resource. Arelis has been seeing 1-3 pts/day. People from Cabirmas come to her to "take my bp." Possibility: set up HTN program ONLY for 2 CHWs in Cabirmas -Supervisor: should be expert on DNHD. Goal should be for CHW's to be having small groups with community on educational sessions, as well as facilitating sessions with the CHW's themselves. One of the requirements is to have internet access.
-Teacher good, nice but doesn't have PH understanding.
-Dr.: can't commit long term but would like to help with trip in March
-Perdomo: has experience with CHWs, interested in small group teaching sessions. She said all CHW projects she has worked with have always been voluntary. Seems like good candidate but we need to find out about relationship with Dra. Also, is she all talk or can she walk the walk? -In Hoyos, sheets are from the community and get misplaced - we should
-Linda: update about nutritional program. Clinic has generator and can now also give immunizations. We will be getting fichas as they come. -feeling from DOH is that we have made a large impact in the villages. Goal is to have us help with other communities once our current communities are up to par.
-HIV testing: DOH has rapid tests and are willing to do. Are willing to do before our clinic. through regional office, not Dra. Perdomo. HAART therapy through La Pina and Dra. Perdomo -laboratory at church - Padre no longer at Parroquia. Trying to get birth certificates for baptizing, and registering for SENASA. We need to decide our relationship with Parroquia vs Padre - historically they would do labs, store meds, get deals. Dra. has been doing-who is paying?? -Water project: no water going to Hoyos right now. Parroquia helping with this... -Assessment: Dra. might not be best for goals of MSA. She doesn't keep in contact with us or CHWs. Day 2: -met with Playa Najayo, dates are set. -CHWs know we are coming in Dec or middle of Jan, Dra. also knows dates and will provide list of all HTN patients. -Forms might be "too complicated" for Dra. -new small nebulizer and computer access for Dra. Will need to talk to exec board. -Public Health people should meet MSA people on Dec and Spring trip.
-December Trip:
Dec 26th-30th. Documentation forms for encounters CHWs to fill out after each encounter with HTN. Exact form, translated, show how to fill out, and expectations for f/u. At end of week the form needs to go to supervisor for compilation and summary. -Who is going? Jon, Liz, Mike, Jon? Stef?
