2015 Time Sheets Instructions  New time sheets should be completed in the same manner as the old Time Sheets. The only major change on time sheets is the addition of the “PLAN OF CARE” section.  The Plan of Care section is to be completed on a daily basis. Every service that is completed for client should be circled in the Plan of Care section.  Blue or black ink only on time sheets – NO Pencil or White Out  Original Timesheets ONLY!! No faxes, emails or copies will be accepted.  Timesheets are only for 2-week Pay Periods.  NO “scratch-outs” or corrections on timesheets without Supervisor approval.  Late Timesheets will be processed on following Pay Period
