4-H HORSE CAMP 18-21 CAMPER REQUIREMENTS
WVU Extension Service has implemented a new policy for 2017 requiring campers that are 18-21 to complete a vetting process. You will find attached a document that will outline all the requirements for campers 18-21 and the memo from WV Extension Service explaining this new policy. You must complete these requirements and the camper packet in order to attend this year’s camp. Southern District 4-H Horse Camp is scheduled for the week of June 18–24, 2017. Title IX mandatory training (explained in the requirements) - Putnam County will be offering two training dates on Saturday, May 6th or Sunday, May 7th from 2-4 at the Putnam County Extension Office (Old Putnam County House), Winfield, WV in the old courtroom. Southern District 4-H Horse Camp Required Camper (18-21) Documentation 1) Volunteer Application Campers (18-21) are required to return a completed volunteer application. The information on this form will be used for the background check (discussed below). Please note that you must list a current email address on your volunteer application to be used for your background check. 2) Code of Conduct Campers (18-21) are required to complete the code of conduct. 3) Background Check Campers (18-21) are required to complete a background check (18 year olds are exempt). The background check is good for 3 years. To complete this process you are required to have a valid email address. The email address listed on your Volunteer Application is the email address that will be used to start your background check process. Extension will submit your background check to TalentWise once you have submitted the Volunteer Application. When your email is submitted, TalentWise will then send you an email with instructions on completing the background process. TalentWise is the company that WVU has selected to conduct all background checks. All background checks must be done through TalentWise even if you have had a background check done elsewhere. THERE WILL BE NO COST TO YOU FOR THE BACKGROUND CHECK. Once TalentWise has completed your background check, the results will be sent directly to WVU Extension in Morgantown for review. No one on SDHC staff, including me, will see your background information. Only the Extension Specialist(s) who are working with this process will see the results. 4) Reference Letters The extension office will mail reference letters to the two references listed on the volunteer application. No immediate family members should be used as a reference. Please let your references know that they will be receiving a reference form and please ask them to return the form quickly. 5) National Sex Offender Registry Check The extension office will perform a check on the National Sex Offender Registry.

Southern District 4-H Horse Camp 6) Children on Campus Training Campers (18-21) are required to complete the children on campus training and the online quiz. After completing the quiz, save the PDF Certificate to your local computer for your records and email the PDF certificate of completion as an attachment. Please send this email to me or Dawna (see below for email addresses). Training: http://employees.ext.wvu.edu/r/download/202613 Please note that you cannot go back to print or retrieve your certificate once you close the PDF. You will need to take the quiz a second time to reproduce the PDF certificate. 7) Title IX Training (New 2017) Campers (18-21) are required to complete this in-person training conducted by WVU personnel. This training is good for 3 years. Please contact your local extension office or the Putnam County Extension office for training dates. All documentation will be on file with Putnam County 4-H Extension Office. It is highly recommended that you scan all completed required documentation as a PDF and email the documentation to Jeff Takarsh (

[email protected]) to ensure that we received your documentation before camp. You can return documentation to the Putnam County Extension Office, in person or by mail.
