District TB clearance dates and spaces are limited. It is your responsibility to have this done in a timely manner. Upon completion of the TB Risk Assessment Questionnaire (with the Nurse): if there are no follow up items needed, the employee will be cleared. If there are follow up items needed, the Nurse will outline clearance steps with the employee. If HR has documentation on file stating that you have had a positive skin test followed by a negative chest x-ray, you do not need to have another x-ray. Call Human Resources @ 644-8000 to make an appointment for TB clearance with a GUHSD Licensed Health Care Provider (Nurse). DATES MAY BE SUBJECT TO CHANGE. PLEASE CALL THE DISTRICT OFFICE FOR AN APPOINTMENT AT 644-8000. Updated 9/12/17
