Midwest Health Equity Conference: Evidence-Based Solutions and Systems-Level Changes for the Prevention and Treatment of Hepatitis/Liver Cancer and Colorectal Cancer Friday, May 12, 2017 | University Center | Chicago, IL Conference Agenda Goal The goal of the Midwest Health Equity Conference is to provide an equal exchange of information and dialogue that focus on enhancing equity and access to care for patients with Hepatitis B (HBV), Hepatitis C (HCV), Liver Cancer, and Colorectal Cancer (CRC) in Chicago and the greater Midwest region. This educational intervention will present the most current, evidence-based information on the prevention and treatment of the target diseases, as well as health equity issues and solutions with the potential to increase access to care and decrease health disparities. An overview of the Conference’s topics/themes of collaboration and improving health equity in Chicago and the greater Midwest region, in addition to the following learning objectives: Recognize the epidemiology and risk factors of HBV, HCV, Liver Cancer, and CRC; Describe barriers to access to care, screening, and treatment for HBV, HCV, Liver Cancer, and CRC for communities of color; Explain advances in the diagnosis and treatment of the target diseases for diverse, at-risk populations along with the relevant limitations and challenges; Identify local and regional peers across community and clinical settings who can enhance patient education, screening, and treatment for HBV, HCV, Liver Cancer, and CRC across the continuum of care; Develop practical action plans to enhance education, access to care, screening, treatment, and engagement with care for use within their individual healthcare settings or communities. Summary: Each track will have the opportunity to participate in follow-up, group discussions regarding prevention, screening, and care/treatment of Hepatitis, Liver Cancer, and CRC. Individual Expert Sessions- Engagement Strategies for At-Risk Communities (Track 1- HBV/HCV/Liver Cancer & Track 2- CRC)

Summary: Participants of each track will gain basic understanding of challenges and solutions involving implementation of evidence-based interventions to increase screening and access to care in at-risk communities. Summary: Each track will have the opportunity to participate in follow-up, group discussions regarding use of data, assessment, and reporting strategies used in various communities to enhance screening and access to care in at-risk communities. Working Lunch & Plenary Session- Health Information Technology to Improve Screening in Community Health and Hospital Systems

Summary: Both tracks will join together to learn about health system characteristics and evidence-based interventions to improve Hepatitis and CRC screening as well as the steps and processes to evaluate their own systems and plan for implementation. Expert Plenary Session- Cancer Treatment & Future Prospects in Liver and Colorectal Cancer

Summary: Both tracks will be provided with information on recent developments in the diagnosis and treatment of Liver Cancer. Additionally, attendees will gain a basic understanding of a multidisciplinary approach to managing and ensuring a complete continuum of care for cancer patients. Joint Group Discussion- Sharing Themes & Lessons Learned from Previous Individual Sessions

Summary: Participants will share lessons and information from each individual track/session, in addition to discussing what the future holds for public health interventions at the local, regional and national levels. Summary: Participants will learn about and discuss possible solutions to ensuring health equity in the screening and access to care for Hepatitis, Liver Cancer, and CRC.
