Attention 6th Grade Parents – Immunization Requirement Prior to Entering 7th Grade! The Ohio Department of Health requires that all students be immunized with a Tdap booster (Tetanus, Page 1

Shaker by Starlight, 6pm & 7:15pm @ HS

diphtheria, acellular pertussis booster) prior to entering the seventh grade. The law requires that students who do not meet immunization requirements be excluded from attending school until they have provided written proof from their health care provider of having received the immunization. If your child has not has not had a Tdap, contact your health care provider or schedule an appointment at the Shaker Heights Health department (216-4911480). Be sure to obtain written verification that your child has received the immunization and submit it to your school nurse. Keep a copy for your own records. If your child has already received the vaccine, check your copy of the immunization record. Your child’s immunization record must indicate that the vaccine is Tdap (OR Adacel or Boostrix which are brand names for Tdap). If the record says Td, DTAP, DTaP, or it is unclear which vaccine the child received, it cannot be accepted. If the record says Td, STAP, DTaP, or it is unclear which vaccine the child received, you must get written clarification from the health care provider that indicates Tdap was given. Immunization records can be turned in to the Woodbury nurse prior to June 8, 2012, and to the Middle School nurse after that date. Please Note: It is suggested that a parent deliver the paperwork to the school. Even the most responsible students often misplace or forget to turn in paperwork. If you have questions, feel free to contact Ms. Madeja, Woodbury School Nurse (295-4155), or Mrs. Mohr, Middle school Nurse (295-4112).
