Following SFC approval for action, this form is used to record changes executed by District and SFD This form replaces forms SFC 705, SFC 706, and SFC 708, which are obsolete Instructions Select district from the drop down menu Input AiM property number (four digit county and district - three digit site - four digit property) Choose property type; input common name and street address for property if applicable Select one checkbox for type of update requested and then input required information Input requestor information: name, title, phone number, email, and date Attach supporting documents (deed, floorplan drawing, etc.) and forward to SFD Routing
