Six Attendees or  Five attendees including an O2 certified person and a DAN O2 kit at the dive site. 

Please submit your completed form to the Treasurer. Any board member may sign or send unsigned form to the  Treasurer. Completed forms may be emailed to [email protected] , hand delivered to the Treasurer  or mailed to the Club at MBDC PO Box 31163 Seattle, WA 98103  The Club keeps air Cards for UWS and Lighthouse in stock. We prefer that you select one of those shops as we  get a discount on our card purchases. If other is selected please purchase the air card from your preferred shop  and submit a receipt for reimbursement. The Club pays for air cards only.  No Nitrox cards.
