Birthday Book Club Celebrate your child’s birthday by donating a book in his/her honor! 1. Complete and return this form with $15.00. Please submit one form per student. Feel free to send cash or make checks payable to the Forest North Library. To accept checks, RRISD policy states checks

must include a phone number and valid

driver’s license number. 2. Your child will choose from a special batch of brand new books to become their Birthday Book. A receipt will be returned to you via your student's take-home folder with a thank you note. 3. Your child’s Birthday book will receive a custom bookplate, and s/he will be the first person to check out the book! The book will become a part of our library collection. 4. A special photo of your student and his/her Birthday Book will be featured on Forest North Morning News Broadcast to thank them for their participation.
