INDIVIDUAL ACCOUNTABILITIES Authorised to deliver the accountabilities required of the Clinical Nurse Specialist including meeting key performance indicators established annually with the line manager. Expertise in clinical practice/ case management accountability   

Accountable for organisational contribution and results. Provision of advanced clinical care incorporating nursing and medical care of the highest standard which is safe and appropriate, in a range of settings that can include nurse led clinics. Provision of effective management of a defined patient group/ area of specialty practice, through direct care and/or supporting others to provide this care, or a combination.

Demonstrates effective clinical management of rapidly changing/ crisis situations.



Demonstrates skilled team leadership with patient group referrals and transport.



Use clinical audit and other evaluative tools to record, monitor and report on the health performance of the specific area of practice.

Demonstrates effective interpersonal skills and commitment to family-centred care.

Assists managers and clinicians with the development and implementation of strategic and operational service plans addressing specific areas of practice.



Demonstrates use of nursing and medical research in clinical practice commitment to nursing education and professional development.

TEAM RESPONSIBILITIES Quality and Patient Safety collective responsibilities  Support the strategic quality objectives within their area of responsibility.  Ensure care is safe, effective, compassionate and dignified.  Foster a culture of openness and improvement, with patient and whanau at the centre.  Develop good multidisciplinary team working and networks to ensure delivery of good quality care.  Ensure the team work to up to date policies and guidelines.  Ensure an annual clinical audit plan is implemented to provide evidence of improvement and good patient outcome.  Ensure that clinical teams have active and effective mortality and morbidity reviews.  Identify and manage risks to the quality of care.

Review and monitor progress through the use of key quality indicators / performance indicators and take action to improve as needed. Receive patient feedback – national inpatient survey, complaints, feedback cards, compliments and act on areas where improvement required.

ORGANISATIONAL RESPONSIBILITIES         

Complies with the Waikato DHB Corporate Records Management policy requirement to create and maintain full and accurate records Can readily identify Māori health need, access rates and equity of intervention and health outcome relative to non-Māori. Promotes a quality practice environment that supports staff to work in partnership with Māori patients and whānau to provide culturally responsive and appropriate care and support to improve health outcomes and reduce health inequities To participate in and comply with the requirements of the Health and Safety at Work Act 2015 and associated Waikato DHB policies. Leads by example and ensures compliance with the established Health and Safety frameworks and systems for achieving best practice in hazard management within own department Communicating to staff through orientation, meetings, training, policy and documentation usage, their role with hazard management in the work place

Professional accountabilities Domains determined by the Nursing Council of New Zealand Domain One: Professional responsibility. This domain contains competencies that relate to professional, legal and ethical responsibilities and cultural safety. These include being able to demonstrate knowledge and judgement, and being accountable for own actions and decisions, while promoting an environment that maximises client safety, independence, quality of life and health. Domain two: Management of nursing care. This domain contains competencies related to client assessment and managing client care, which is responsive to the client/ clients’’ needs, and which is supported by nursing knowledge and evidence based research. Domain three: Interpersonal relationships. This domain contains competencies related to interpersonal and therapeutic communication with clients, other nursing staff and interprofessional communication and documentation. Domain four: Interprofessional health care and quality improvement. This domain contains competencies to demonstrate that, as a member of the health care team, the nurse evaluates the effectiveness of care and promotes a nursing perspective within interprofessonal activities of the team.

QUALIFICATIONS AND EXPERIENCE QUALIFICATIONS    

Registration with Nursing Council of New Zealand; inclusive of scope of practice. A current practising certificate with the Nursing Council of New Zealand. Significant clinical experience, with the last two years being in the specific specialty. Expected to have net expert level of practice on the Professional Development and Recognition Programme, but may achieve a higher level of practice. Completed a post graduate qualification within the specific specialty.

 

WORK ENVIRONMENT AND WORK FUNCTION / ACTIVITY Work environment 

Works indoors within hospital wards, departments, health clinics, community settings and their offices.



Employees work in adequately lit, heated, ventilated and clean, well maintained and hygienic workspaces.

Work function/activity 

Medium to heavy physical demand, depending on the work setting.



Sits occasionally during patient briefings, writing reports and at staff meetings.



Stands and walks frequently to monitor and examine patients, gather and use equipment, medications and deliver other medical interventions.



Lifting up to a heavy demand will be required when moving patients to carry out nursing procedures.



Occasional bending, squatting, crouching or stretching, twisting and reaching may be required when carrying out nursing procedures.



Frequent contact with body fluid and materials will necessitate the use of protective clothing and gloves and safety procedures will be adhered to.



Occasional repetitive hand and finger movements will be necessary when using computers and writing notes.



There will be frequent use of surgical and medical equipment and materials including medicines, medical instruments, haemodynamic monitoring equipment, medical dressings and sterilising equipment.



Mental activities necessary include a high level of cognitive functioning with nursing, observation, evaluation, assessment, communication, interpersonal, organisational, problem solving and decision-making capabilities. Source: www.acc.co.nz
