Emergency Medical Treatment: In the event that either a parent or an emergency contact person cannot be contacted by telephone, I authorize Cashton High School personnel to use discretion and seek medical attention. I also give permission to authorized medical personnel to give first aid and emergency treatment to my child should they require such assistance. ___________________ Student

Student Athletic & Extra Curricular Handbook: I have read the rules and policies set forth for athletic participation at Cashton High School and give my child permission to participate under these conditions.
