Responsibilities as a Clinical Pharmacist at SKMCH&RC Lahore Pakistan. Worked on Completely online system in the Hospital. As a Clinical Pharmacist in Provision of Clinical Pharmacy services to ICU, Medical Oncology (adults), Medical Oncology (paeds), Internal medicine and Surgery Patients. Attend rounds with Med-ONC Team . Identify Pharmacotherapy related Problems, Assess the severity & Significance of the Problems and present evidence based solutions to these problems to Medical team. Design, Implement and Assess the Patient care plan for Patients in Collaboration with Medical team. Participate in Research projects with Medical team. Design, Define & Implement Protocols & guidelines with Medical team. Clinical Pharmacy Management. Drug Information Center management.



Monitoring/ Screening of the patients to raise their pharmaceutical care issues hence catering for better patient care.

Reporting of Adverse Drug Reactions, Drug Interactions, and Clinical Interventions, furnishing therapeutic choices to Consultant Physicians and following/checking investigations of the patients to correlate with therapy.





I have complete command on the knowledge of various cytotoxic agents, Various Target therapies (Like Monoclonal Anti-bodies), Chemotherapy protocols for various malignancies, knowledge of Clinical Pharmacy aspects of Hematology and Oncology.



I check and verify chemotherapy protocols, Body surface area (BSA) of the Patients, AUC of the chemotherapeutic agents, standard dosages of various chemotherapy protocols. Adjustment of the drug dosages if needed and helped in the management of Chemotherapy induced complications such as Myelosuppression, Febrile Neutropenia, Renal Failure, Diarrhea, Vomiting etc and kept the record of chemotherapy administration time in order to know whether patient is in Nadir or approaching Nadir or coming out of Nadir.



I helped the Consultant Physicians in the selection of various antimicrobials to treat various infections in Immunocompromised (Febrile Neutropenic) and immunocompetent patients.



I played a vital role in Peripheral Blood Stem Cell Transplant (Autologous PBSCT) at SKMCH & RC.



Full command in adjustment of dosages of drugs in Renal, Hepatic, Cardiac impairment and any other organ impairment.



Full command on Clinical Pharmacy in Geriatrics, Pediatrics and Adult medicine.



Total Parenteral Nutrition (TPN) prescribing and monitoring of the patients those who are getting TPN in order to assess daily requirements/ Adjustment of the ingredients (Macro- and Micronutrients) as well as response of the patients. I also have full command in calculation, manufacturing and dispensing of TPN



Therapeutic Drug Monitoring and Pharmacokinetics.



Clinical Pharmacy services are rendered to Medical Oncology (Adults and Paeds), Radiation Oncology, Internal Medicine (Pulmonology, Gastroenterology, Infectious disease, Neurology, Urology, and ICU), Surgery, Palliative Care, and Clinical Research Department and Stem Cell Transplant (Autologous PBSCT)


