DES MOINES AREA COMMUNITY COLLEGE NURSING PROGRAM PRACTICUM EVALUATION The practicum evaluation tool is used to document the student’s progress towards meeting practicum competencies for the course. The evaluation is applicable to all practicum settings where a clinical instructor is present including, but not limited to, acute care, long-term care, and community. W ​ hen attending simulated clinical experiences, a tool specifically designed to evaluate students in simulation will be utilized. The simulation evaluation tool scores will be incorporated into the student’s summative practicum evaluation. The practicum evaluation tool is divided into ​formative​ and ​summative​ sections. The emphasis during formative evaluations is on monitoring progress/improvement towards achievement of criteria, whereas summative is a final evaluation to determine if performance criteria have been met. The student is expected to retain the skills and abilities of performance criteria achieved in previous courses. FORMATIVE EVALUATIONS will be completed once (1 time) per week/practicum setting. If the student is in more than one (1) practicum setting in a week, a formative evaluation will be completed for each practicum setting. Additional formative evaluations may be completed at the discretion of the instructor. Formative evaluations include student self-evaluation and instructor feedback. The performance criteria for the formative part of the evaluation tool are: “SP”

(Satisfactory Progress)​ – student demonstrates steady, consistent performance related to identified clinical competencies. Student will receive the number score associated with the competency.

“NI”

(Needs Improvement)​ – student needs to demonstrate more independent performance of clinical competency on a continuing basis. Student requires much or repeated assistance/redirection and requires verbal and/or physical cueing from instructor. Student will receive a zero (0) for the competency.

Total score and percent will be calculated for each formative evaluation. A percentage of 79 or less requires a remediation plan. Remediation may be required at the discretion of the instructor at any time regardless of the percentage. The SUMMATIVE EVALUATION will be completed at the end of the course. Percent for each formative evaluation will be totaled and divided by the number of formative evaluations in the course. In order to pass the practicum component of this course, the student must achieve a total average percentage of 78 or greater. A total average percentage of 78 or less will result in a failure for the course. The performance criteria for the summative part of the evaluation tool are: ​“S”

(Satisfactory)​ – total average of 78% or greater

​“U”

(Unsatisfactory)​ – total average of less than 78%

Completion of the current course requires meeting both practicum competencies and theory components successfully.

During my experience with my patient at the acute care setting I came across a barrier to communication that made me change how I normally interact with a patient. Normally I am able to ask open ended questions, but with my patient they were only oriented to person and their speech was mostly unintelligible. They were able to answer yes and no easily enough so I had to switch to more direct questions. I also had to simplify and shorten my sentence because the patient had a hard time staying focused on me long enough if they went on too long. I also found that lowering myself down to be eye level with them and making sure they were focused on me before talking helped them understand what I was saying better.

My patient was in droplet precautions, so I made sure to wear a mask every time I went in. My patient also had a colostomy bag so I made sure to check it routinely to make sure it wasn't filling up with gas, and I also made the patient care tech aware of the colostomy bag. I reported to the nurse I was with about the sores appearing behind the patients right ear, and I applied some cushions to the nasal cannula to help prevent them from getting worse.

Going back to the nasal cannula that starting to create a sore spot behind the patient's right ear I found that during the assessment portion of the nursing process. I then used that information, combined with my instructor letting me know that it would be a good thing to cushion that area, to create a plan to get some gauze/padding to tape around the nasal cannula. I then implemented that.

I think the biggest thing I noticed was that there were two patients that were unable to move there lower body. In one case it was a younger adult, and the other was an older adult. I noticed when the nurses went into the room of the younger adult they allowed them to move do things for themselves a lot more, and even allowed them to transfer themselves to their wheelchair. On the other hand with the older adult they were assisted with being transferred, and a lot more actions seemed to be performed for them or at least helped with.

Got to participate in a nurse shift change. Saw bedside report being done, which I found helpful to put a face with each patient while discussing their needs/diagnoses. During an assessment I was able to act professional in the sense that I respected the patients right to privacy when the maintenance/cleaning staff walked in by not exposing certain areas of the patient that I normally would for an assessment.

I was able to have a discussion with a patient about their spiritual beliefs. The patient was a youth group leader, and I was able to give them ideas from back when I used to be part of a youth group on trips we took. I also noticed while talking with the patient about their beliefs it was able to distract them from their pain, and their demeanor became more relaxed.

I think one of the best things I learned was how my instructor described teaching a patient about colostomy care. I was told to relate cleaning the colostomy bag to wiping after having a bowel movement so that the action doesn't seem quite as foreign to the person. I also liked how my nurse I was following would ask a patient what they normally do for their blood sugar at home when it gets low. I think it definitely helps promote health and gets the patient involved when asking them what they do at home for certain things.

Student Identified Strengths: I was willing to ask questions about why certain things were done the way they were. I made sure to ask not only if nurses needed help with something but also the patient care techs. I am quick to resort to asking an instructor or a nurse why something is done, rather than trying to find the answer myself. I also need to work on delegating, and possibly learning the ability to say no to tasks that don't necessarily need my level expertise.
