COURSE OF STUDY APPROVAL FORM FOR APPROVED EXCHANGE PROGRAMS ATTENTION STUDENT: This form serves to approve your expected schedule of courses from your exchange program experience. To complete the form, take a syllabus, if available, or course description of each class to be taken abroad to the Office of International Programs & Services to determine the tentative transfer course equivalency. NOTES: 1) Final determination of transfer credit will be based upon review of course materials provided by the student and transcript provided by the host institution. Any credit awarded follows Georgia Southern University’s process for awarding transient credit, as published in the Undergraduate Catalog. The Student, the Office of International Programs & Services, and the Office of the Registrar will maintain a copy of this form. 2) It is the student’s responsibility to:

1) Keep all course materials including syllabi, course descriptions, coursework, and textbook information, necessary for evaluating transfer credit upon return to Georgia Southern. This also applies to courses that have specific host university pre- enrollment requirements such as language placement testing. 2) Submit course materials, included in #1 (above), to the Office of International Studies within 10 business days of completing the term. 3) Know that if he/she is within 25% of hours needed to graduate, the student’s department chair and dean (of major) must approve the form.
