P­CCS Student Chromebook User Agreement (PCEP)  Definitions:  1. P­CCS means Plymouth­Canton Community Schools.  2. Device means the laptop, tablet, phone or other device, the charger, and any accessories provided by P­CCS.  3. Student/Parent means the P­CCS student and parent(s) to whom the Device​  ​ is assigned.  4. School means any location on P­CCS property or under P­CCS​  ​ authority,​  ​ as well as any other private or public location  where P­CCS­related work is being conducted.  5. Policy means all applicable laws, regulations, P­CCS Board policies, and P­CCS Administrative guidelines, including the  P­CCS Technology Acceptable Use policy.   

Terms and Conditions:

 

1. The Device is and will remain the property of P­CCS.  2. The Device is intended only for use by Students.  3. The Device is assigned to Student for school­related work. Student agrees to use the Device for school­related work as  defined by the Policy.  4. Student/Parent bears full responsibility for all use of the Device and agrees to ensure that all use abides by the Policy at  all times and in all locations.  5. There is no expectation of privacy for any use of the Device. P­CCS may monitor any use of the Device, but does not  guarantee that any use will be monitored.  6. If Student/Parent has met P­CCS requirements related to equipment liability (*see below for options), the Device may be  taken home or to other locations by Student. However, the Device should be available at the School whenever needed to  support School­related tasks.  7. Student/Parent agrees to take reasonable precautions to keep the Device safe and in working order. If the Device is lost,  stolen, or damaged, Student/Parent agrees to promptly report the incident to the P­CCS Technology Department and law  enforcement authorities as appropriate.   8. P­CCS will not assist Student in connecting the Device to non­P­CCS networks.  9. Student agrees to promptly return the Device to P­CCS when requested. Failure to return the Device is subject to the  Policy for P­CCS property.   

Participating families are required to choose only ONE of the following three liability options:   

Insurance Premium​  ​ Purchase​  ­ Non­refundable payment​  ​ in the amount of $30​  ​ made through PaySchools  links.pccsk12.com/premium​  by Tuesday Sept. 8, 2015.  ​ Under the terms of the insurance agreement, in the event the device  needs to be repaired or replaced due to accidental damage, non/out­of warranty issues and theft, an approved claim carries  with it a deductible to be paid by the student's family. If the claim is denied, the family is held financially responsible for the  full cost of the repair/replacement needed. ​ Student/Parent accept responsibility for any/all deductibles (in the event of device  damage, out­of­warranty repair or a police­reported theft, a deductible of $35 would be paid; in the event of loss without  police report and approved claim, a deductible of $100 would be paid). ​ For a general policy overview, please see  http://links.pccsk12.com/gocare​  Please note, for families with more than one student participating in device take­home, there  is a sliding scale for insurance premiums.  First child ­ full premium ($30), second child ­ 65% premium ($19.50), additional  children ­ free.  Deductibles are still charged at the same per incident rate as listed above.    

Insurance Premium Reduced/Waived due to income eligibility​  ​ The P­CCS Board of Education has endorsed a policy  that waives the device insurance premiums for families eligible for free priced meals with the National School Lunch  Program, and a reduction of device insurance premiums for families eligible for reduced priced meals with the National  School Lunch Program.  ​ Student/Parent do however remain responsible to pay the deductible charges listed above in the  event of theft, loss or damage.    ● If you have been approved for free lunch, there is no action you need to take regarding the insurance premium  payment.    ● For the 2015­2016 school year, the insurance premium cost for ​ students eligible for reduced­priced meals is $6 for  the family’s first student participant, $3 for the family’s second student participant and free for additional students  participating in the program.​  For participating students that are approved for reduced lunch, please select the  appropriate payment option at ​ http://links.pccsk12.com/premium​  by Tuesday Sept. 8, 2015.     

Refundable Deposit ­ ​ $235 Chromebook deposit made through PaySchools ​ http://links.pccsk12.com/deposit​  by Tuesday  Sept. 8, 2015. Should the device become damaged, lost or stolen, funds up to the entire amount would be deducted from this  deposit to address the cost of repair/replacement. Any funds remaining on deposit would be refunded after the device is  returned. If the device is returned completely undamaged, the deposit in its entirety will be refunded.     (over)

Depending on the P­CCS equipment liability option chosen, my family  is fully responsible for the insurance deductible or the forfeiture of part  or all of a deposit to address the cost of repair/replacement should my  device become damaged, lost or stolen. ​ W e also accept responsibility  for lost or damaged device accessories (ie. adapter and sleeve).  I will not leave my device unattended unless it is locked in a secure  place.   I will protect the device whenever it is moved from one point to  another.  I understand that my device can sometimes be monitored remotely by  software installed by the district, but that parent(s) / legal guardian(s)  are primarily responsible for monitoring appropriate use of the device  outside of school.
