Give Online Consent for Immunisation and Dental Services To support your child’s wellbeing, the Health Promotion Board provides free health services, such as screening, immunisation and dental checks, at specified times during school hours in schools. Parents must give their consent before their children can receive such services. Every year to date, over 99% of parents have done so. To make it more convenient and secure, we have launched a new portal to submit your consent online. It only takes three steps: 1. Log on to https://childconsent.hpb.gov.sg using your SingPass with 2-factor authentication (2FA)

after 21 Dec 2016. You may register for SingPass by visiting https://www.singpass.gov.sg OR any SingPass counter. The location of SingPass counters and instructions for 2FA activation are available on

https://www.singpass.gov.sg. 2. Type in your child’s NRIC number

3. Fill up and submit the medical form Please submit your consent online by 15 Jan 2017. You may contact the school should you require a hard copy of the form instead. Thank you.
