Healing Check and Reevaluation You have completed the root planing. Deposits and bacterial plaque have been removed from the roots below the gums. With the irritants gone you will heal. As the inflammation decreases, the pocket depths around the teeth become shallower and more cleansible. The better you brush and floss, and if asked, proxabrush and toothpick, the more you will heal. We ask you to “not miss anywhere” once per day. The bacteria that cause gum disease are mostly anaerobes. “An” means “no.” “Aero” means “air.” So anaerobes can’t live where there is air or oxygen. When you floss or proxabrush you oxygenate or aerate between the teeth and under the gums. So if all pockets have cleansable depths, and you daily clean everywhere, the bad germs can’t live anywhere. If deeper, uncleansible pockets persist, surgery is indicated. About four weeks after the last root planing you have a “healing check” appointment. A dental assistant checks that everywhere is healing properly. If inflammation somewhere is not resolving, it is usually because you are missing the site. Please bring all your tools with you to the healing check, so the dental assistant can demonstrate any improved techniques to achieve maximum healing. By the time of the healing check tissues have often shrunk enough that a larger proxabrush is more effective. The most common deficiencies are not using the proxabrush from the insideout, not getting low enough inside of lower teeth with the brush, and not wrapping enough with the floss to go below tissue on the inside corners. About eight weeks after the last root planing you should be maximally healed. All inflammation/redness/bleeding should be resolved. You are ready for the reevaluation with the doctor. We again measure the pocket depths. Three and four millimeter pockets are readily cleaned by proper use of your oral hygiene tools. Five millimeter pockets are borderline cleansable with excellent techniques. Sixmillimeter and deeper pockets cannot be cleaned or controlled by you, so surgery is usually indicated to reduce the pockets to depths that are cleansable.
