HEALTH ROOM MEDICATION PROCEDURE Note the following: 1. All medication must be brought to school in the original container by a parent or responsible adult. 2. The medication container must be accompanied with a Medicine Drop Off form completed with the number of pills and parent/adult signature. 3. Prescription drugs must be accompanied by the Permission to Administer form signed by both parent/guardian and physician. 4. Changes in dosages or times of administration must be documented with a new Permission to Administer form signed by the parent/guardian and physician. 5. It is the responsibility of the student to come to the Health Room to take the medication at the proper time. 6. Non‐prescription medication such as Aspirin or Tylenol must be in the original container, and accompanied by a note, signed by the parent giving the name of the drug, dosage, and time to be administered. A NEW NOTE IS REQUIRED FOR EACH DAY. If a parent requests that a supply of a drug such as Tylenol, Advil, etc. be kept in the health room, and administered at intervals throughout the year, the prescription drug procedure must be followed, requiring a Permission to Administer form signed by the parent/guardian and physician.
