Kit includes: 2 sets of blue & gray putty, 2 impression trays, instructions, return mailing label, zip-loc bag The follow ing are instructions for you to take a perfect impression of your mouth w ith the at home impression kit and materials provided. Please follow the instructions carefully to produce the most accurate impression possible. IF YOU HAVE ANY OF THE FOLLOWING DO NOT USE OR ATTEMPT TO USE THIS KIT. Loose teeth, bridges, braces/brackets, crowns, veneers. CALL US IMMEDIATELY!! STEP 1 — PREPARATION. remove all items from kit package and place on stable surface. Make sure all items are included in your package. Wash hands thoroughly prior handling any materials. Find a clean stable surface to place all items. STEP 2 — identify which of the impression trays provide the best fit and size for your mouth. Insert each into the mouth making sure that the tray extends to the back molars of the mouth. Place the selected tray down in front of you on a surface. STEP 3 — Unwrap 1 blue putty and 1 gray putty. DO NOT combine or mix these yet. IMPORTANT! — the self forming putty is a fast set putty which begins to harden and form after just 15-20 seconds after mixing. Therefore, be prepared to follow all instructions carefully and work quickly during this process. STEP 4 — When ready, put the blue and gray putty in the palms of your hands and begin to mix quickly together using your fingers and hands for approximately 10 seconds until the putty turns a solid blue color. Marbling should minimal or not be visible at all. Once mixed, roll the putty between your hands to form a tube-like hot-dog shape. IMPORTANT! — for best results, the tube should be thicker toward the middle of the tube and thinner towards the ends. STEP 5 — place the tube-like putty in the tray so that the putty extends from 1 molar, around
across the front teeth of tray and extends to the other molar. The thicker middle part of the tube should be located around the front teeth area of the tray. STEP 6 — gently, with your thumbs push the putty down into the tray to provide a slight indentation of where the teeth will be placed creating a slight ridge toward the outer rim of the tray. STEP 7 — With both hands, grab the stem of the tray with your forefingers and thumbs and place the tray into the mouth. IMPORTANT!! — DO NOT BITE DOWN ON THE TRAY. NO BITING OR CLENCHING OF TEETH SHOULD OCCUR IN THIS PROCESS. Put tray as far back of mouth to cover molars. The top lip should come over the front of the tray so that the tray is fully in the mouth. With the forefingers holding the stem, thumbs should be placed on the bottom of the tray and used to push the tray up into the teeth. Be sure to use the thumbs to push as evenly as possible from the front to back on both sides. You should see putty pressing through the slots and holes of the tray. Once pushed up into the teeth, thumbs and forefingers should be use to continue to push and hold the tray in place for 3 minutes. minutes STEP 8 — after 3 minutes, pull down on the stem of the tray to dislodge from the mouth. Inspect the impression to make sure all teeth are defined, the gum line can be seen, and back molars can be seen sitting within the tray. YOU MAY snap a close-up picture of the impression and email to
[email protected] for evaluation of a successful impression prior to returning. STEP 9 — put tray into zip-loc bag provided and lock tightly trying to remove as much air inside bag as possible. Put zip-loc bag with impression as well as all materials provided back into box and tape securely. Adhere return shipping label
