Independent P.E. Contract
A student may receive a half (0.5) credit for 67.5 hours of physical activity accompanied by a 500-word paper. A mentor or coach over 21 years of age must document the activity which must be performed in increments of at least 30 minute sessions. Independent P.E. credit may be applied to the Physical Education requirement or will count as an elective credit if the P.E. requirement has already been met. This course will not fulfill the Fitness Concepts requirement. The course is offered Pass/Fail.
1. Complete this enrollment form signed by the student and the HomeBRIDGE representative.
2. Keep the log form of your hours. They need to be signed by whoever is
supervising your course. If you are on a team those practice hours may count as long as the coach signs the log. Game time does not count.
3. Submit a two page, double spaced, 500-word paper. The paper should explore
an area of physical fitness that you would like to improve such as your heart rate, increased stamina, weight loss, etc. The second page will evaluate whether you did or did not reach this goal. The paper must be clearly written with acceptable grammar and spelling.
