Completing the Online Entry Cover Sheet for the 2014–2015 Alternate Proficiency Assessment (APA) The Entry Cover Sheet is available and ready for use. The required information will be completed electronically, produced as a PDF file, printed out, and placed in the binders. All teachers must use the online Entry Cover Sheets as part of their students’ portfolios. The NJ APA online Entry Cover Sheet is found on the NJ APA Questar website at: https://nj‐servicepoint.questarai.com/Login.aspx The Entry Cover Sheet is a required form that is completed and included as part of the portfolio to document the Content Area (Science only), entry number, Standard, Strand, CPI, CPI Link Type, and the specific CPI Link assessed for each entry submitted for the APA. There are two pieces of evidence required for each entry (evidence of Initial Activity and evidence of Final Activity), and the Entry Cover Sheet provides a space in which you must include a description of each activity as it demonstrates the CPI Link assessed. It is of the utmost importance that the Entry Cover Sheet be completed online with all the required information and included in the binder to precede the entry evidence. The entries must be in the order designated in the test specifications (Entry 1 related to the first Standard and Strand required, Entry 2 for the second Standard and Strand required, etc.) and the entries must be placed in the binder in that order. To complete the Entry Sheet online, you will select information from drop‐down menus after clicking on a down arrow. After you make your selections, the system will auto‐populate the information into the form. Be sure to double check that you have selected the correct information from the list; you may correct the information by changing your drop‐down selection, which will then change the information printed in the boxes. Remember that you must type in the student’s name and the two activity descriptions, as there is no pre‐populated information for these boxes. After all information is entered and a PDF of the form is created, save a copy of the completed Entry Cover Sheet to your computer. Saving a copy of the PDF on your computer will allow you to be able to print it out for inclusion in the binder and/or make any necessary edits. The Content Area will not be selectable as the 2014‐2015 APA will be Science only.
Select the appropriate Grade from the Grade of Record for Assessment drop‐down menu.
This is the grade level associated with the Standard and Strand, which should match the student’s grade level for grades 4 and 8. Students in grades 9–12 will be assessed on Standards and Strands related to completion by the end of grade 12, therefore select High School for these students.
Select Entry # from the drop‐down menu.

Type the student’s full name.
The student name can be typed into the form at any time throughout the process with no effect on drop‐down functionality.
Be certain to type in the student’s full name before saving the form and printing the Entry Cover Sheet for submission in the binder.
The Standard required for assessment will automatically be filled in with the Standard # and description based on the Grade and Entry # chosen.
The Strand required for assessment will automatically be filled in with the Strand name based on the Grade and Entry # chosen.
Select CPI # from the drop‐down menu.
You will be prompted to PLEASE SELECT from the drop‐down menu the CPI chosen for assessment from the eligible CPI #s available based on the Grade and Entry # chosen. Refer to your Procedures Manual if you need to confirm the code of the chosen CPI.
The CPI Link Description will automatically be filled in once the CPI # has been selected.
Select the CPI Link Type assessed (Matched, Near, Far) from the drop‐down menu.
Select the CPI Link Bullet you have chosen to assess.
Select the CPI Link chosen for assessment based on the order of the bullets on the CPI Links document (e.g., Matched Link First Bullet, Matched Link Second Bullet, etc.). Be sure to verify the CPI Link Bullet number against the list of links in the Matched/Near/Far link columns to ensure you are making the right selection. •
Refer to the CPI Links in the 2014–2015 NJ APA Procedures Manual to confirm this information.
The CPI Link Description auto‐populates once the CPI Link Bullet is chosen. Please verify that the link description is accurate based on the CPI Link you are assessing. If this information is not accurate, return to the CPI Link drop‐down menu and revise your selection based on the correct bullet number.
Type the Activity Description for the First Collection Period (Initial Activity) and the Activity Description for the Second Collection Period (Final Activity) into the boxes provided.
Each activity description should describe the activity as it demonstrates the CPI Link.
All parts of the CPI Link must be assessed and should be addressed in the activity description.

Adobe Reader 9.8 (or higher) is required in order to print or save the online Entry Cover Sheet after it is completed.
If you are using an older version of Adobe Reader, you can obtain an updated version by going to: http://www.adobe.com/downloads/; click on the Acrobat Reader free download link on the right‐hand side.
Your browser must be configured to allow pop‐ups in order to print or save the PDF file.
Once all information is entered, click the Submit button on the bottom of the screen. Clicking Submit will open a new window on your computer with the final Entry Cover Sheet in PDF format. The PDF file will include all information entered into ServicePoint as well as a date and time stamp in the upper right‐hand corner for reference. As a note, clicking on the Submit button does not electronically send the Entry Cover Sheet to Questar. Please procede to the next step to save your Entry Cover Sheet.
The Entry Cover Sheet should also be saved to your desktop or local network. After saving the Entry Cover Sheet to your desktop, you must open the Entry Cover Sheet with Adobe Reader or Adobe Acrobat. If you do not have either of these, please navigate to Adobe’s website and download and install the Adobe Reader tool at http://get.adobe.com/reader/. Adobe Reader is offered free of charge.
Should you wish to edit the PDF file at a later time on your desktop, you will be able to do so. The PDF that was created is editable.
Print out the Entry Cover Sheet and include it in the binder in front of the evidence for each entry submitted.
After confirming the PDF file is saved to your computer, close the “new window” that opened after you submitted the form. You should now see the original window, where you entered the Entry Cover Sheet information.
Click the Clear Form button on the top of the ServicePoint screen to clear out all information (including both typed and auto‐populated), reverting the screen to blank fields.
You may now generate another Entry Cover Sheet, and continue the process as already described.
Click the Cancel button at the bottom of the ServicePoint screen when you are finished completing all students’ Entry Cover Sheets, in order to return to the Documentation page on the NJ APA ServicePoint website.
