Laboratory Safety Contract The following rules are designed to ensure safety in the laboratory. It is necessary to always follow these guidelines and always exercise proper behavior when performing an experiment. Read these rules with your parents and then sign the bottom of the contract. Please keep this copy of rules in your Science Survival section of your notebook!

I, _____________________________________, agree to abide by the following laboratory safety regulation whenever performing a science investigation. Dress Code: 1. Always wear safety goggles 2. Always tie back long hair 3. Tie back or remove loose clothing and/or jewelry General Safety Rules: 4. Carefully read all of the lab instructions before you begin your experiment 5. Always maintain a clean work area 6. Do not perform an experiment without the permission of the instructor 7. Only use the equipment designated by your instructor 8. Clean up all spills immediately 9. Never eat or drink in the lab 10. Never horseplay in the laboratory First Aid: 11. Report all accidents to your teacher immediately 12. Know how to rinse your eyes at the eye wash station 13. Know the location of the first aid kit in your classroom Chemical Safety: 14. Never mix chemicals for the “fun of it” 15. Never touch, taste, or smell a chemical unless instructed to do so 16. Never inhale fumes directly. If instructed to note the fumes of a chemical, carefully waft your hand over the chemical and direct the fumes to your nose 17. Only use the chemicals needed in your experiment 18. Keep all lids closed when a chemical is not being used 19. When using acids or bases, always pour such chemicals over a sink and never over your work area 20. When diluting an acid, always pour the acid into the water. Never pour water into an acid. Glassware and Sharp Object Safety: 21. Never eat or drink out of laboratory glassware 22. Never use broken or chipped glassware 23. If glass breaks in a lab, sweep it up immediately with a broom and dustpan and dispose of it in the proper container designated by your teacher 24. Never pick up hot glassware with your bare hands 25. Never heat wet glassware 26. Always cut away from you 27. Use extreme caution when handing sharp objects At the end of your Experiment: 28. Always clean up your work area and return all equipment to its proper location 29. Wash your hands before you leave the laboratory 30. Double check that all burners are off before leaving the classroom

Inappropriate behavior will result in one or more of the following: 1) Detention 2) Referral 3) Zero on the lab 4) Restriction from future labs.
