Media Usage at Lake Mary Toastmasters At meetings of Lake Mary Toastmasters, various media recordings are made. These visual and audio recordings are used primarily for the educational use of our members and guests and secondarily for promotion of our club. We will exclude you from any or all recordings if you wish. You may also request a copy of any recordings of yourself for your own personal use. What follows below is the legal release forms for photo and video recordings. Please sign the form and provide us a copy. Sending us a picture of this form taken with a smartphone is probably easiest. You may send it via MMS (text message) to our Google Voice number at 321-209-3601 or email it to [email protected]. You may keep this original form.

Photo Release Form I ( __ do __ do NOT ) consent and authorize Toastmasters International or any entity authorized by Toastmasters International (such as a Toastmasters club or district) to copyright, use and publish any of the images in any format taken of me at Lake Mary Toastmasters club meetings. I understand these images may be used for a variety of purposes and may appear on the club or district website, the Toastmasters website, in the Toastmaster magazine, promotional materials or any other media now known or to be invented. I also understand that Toastmasters International or any entity authorized by Toastmasters International will use the images exclusively for Toastmasters-related purposes and not for any commercial gain. Since anyone can download an image from the Internet or make copies from printed materials, I agree that Toastmasters International is not responsible for unauthorized use of the images. I am aware that I am not entitled to any compensation and that the images may appear with or without my or my club’s name. By signing below I acknowledge I have read and understand this release.

Video Release Form The undersigned enters into the following agreement (“Agreement”) with Lake Mary Toastmasters (“Producer”). I have been informed that Producer frequently captures video footage of events involving members of Lake Mary Toastmasters. As part of each recording (“Product”) in which I appear: 1. I grant Producer and its designees the right to use the Product in any format, now known or later developed; the right, without limitation, to edit, mix or duplicate and use or re-use Product in whole or in parts as Producer may elect under the following indicated conditions: ______ My name is excluded from any association with the Product ______ My likeness, image, and appearance is excluded from any version or derivative of the Product ______ My voice is excluded from any version or derivative of the Product 2. I ( __ do __ do NOT ) grant the Producer an irrevocable and perpetual license to the copyright of my performance(s) under the terms of the Creative Commons Attribution-Noncommercial license. I understand that I am responsible for the content of my performance, and I shall indemnify the Producer against any claims brought versus the content of my performance. 3. I understand Producer or its designees have complete ownership of the Product including without limitation the right to broadcast, exhibit, market and otherwise distribute the Product, in whole or in parts, and alone or with other products, for any purpose Producer or its designees determine. This grant includes the right to use Product for promoting or publicizing. 4. I have the right to enter into Agreement and am not restricted by commitments to third parties. 5. Producer has no financial commitment or obligations to me as a result of Agreement. 6. In consideration of all the above, I hereby acknowledge receipt of reasonable and fair consideration from Producer.
