2017 Modoc Fire Report Guide

All Fire Reports will have a copy Incident Organizer, and Line Officer Inspection Sheet attached, when turned into Dispatch. Dispatch will retain all documents for record retention purposes.

This Fire Report Guide assists users with completing the Individual Fire Report Form (5100-29) by explaining what information is needed in each field and as a reference to the codes used for entry into FIRESTAT. This guide is organized to locate and reference specific parts of the report instructions quickly. FIRESTAT is a computer program that collects individual Wildland fire information from Federal Wildland Firefighting agencies nationwide and creates a database that can be used by a variety of other computer programs to provide historical data, which is, used by managers, planners and others at local regional and national levels. The information provided in this report along with other data, such as weather, is used as a basis for making a wide range of decisions such as the funding of resources, mobilization, staffing strategy and long term planning as well as reporting on program performance. FSH 5109.14.6 States: Report preparers must enter complete, accurate data into the national fire occurrence database no later than January 15 of the year after discovery regardless of whether or not the fire is declared out (FSM 5183.2). Reports must be correct and complete in the national fire occurrence database according to the following reporting time frames: Report standards (FSM5180) dictate the timeframes by which reports should entered and are as follows: 1. Within 10 days of the discovery date, initiate the fire report in FIRESTAT and complete to the extent that reporting data is stable. (MICC) 2. Within 10 days after the fire’s management strategy is met, enter the acreage and Fire Code data. (MICC) 3. Initial Attack IC or designee Within 10 days of being declared “out”, document and complete the report, including approval from Battalion or Division Chief for submission to MICC.

1. FIRE NAME - Modoc Interagency Communication Center (MICC) will name each fire during the initial attack phase of the fire. The name will be a unique one-word name depicting geographical landmarks taken from the Modoc Recreation map and an incident number generated by WildCAD. (E.g. MDF DH Cougar) 2 .LOCAL FIRE NUMBER - This is the incident number generated by WildCAD when the incident is created. (E.g. CA-MDF-000323) 3. GEOGRAPHIC LOCATION OF FIRE - This is a geographical feature to help locate the fire (E.g. Ash Creek,, Crank Springs, Mowitz Well, etc.) Avoid using directional descriptors such as 3 miles east of Hwy 139 and Dry Lake to describe the location. These are not directions; all the report needs is a general geographical area. 4. LOCATION - Township, Range and Section- No longer needed 5. REGION/FOREST/DISTRICT/SO FIRE NUMBER - Region = 05, Forest = 09, District = (see codes below) the District where the fire occurred. The SO fire number is provided by MICC and is listed on MODOC WILDWEB at http:// /WildWeb/WCCAMICC.htm 53 for WMRD 54 for BVRD 55 for DGRD 56 for DHRD 6. PROTECTING AGENCY AT ORIGIN - This is the direct protection authority (DPA). Which jurisdictional umbrella is the fire under? TIME OF IGNITION - Enter the month, day, year and time of ignition as close as you can estimate it. This is an estimate of the time the fire actually started entered as MM/DD/YYYY-MMHH or 05/07/2015 0600. 11. TIME OF DISCOVERY - Enter the month, day, year and time of discover. This can be found in the WildCAD run report (the time the incident was created). Enter as MM/DD/YYYY-HHMM or 05/07/2003-2100 0800.

For example: H2 is a heavy lift type 1 helicopter not medium lift type 2 helicopter such as CalFIRE H-202.
