Per the Ohio Revised Code 3701.13, all students entering 12th grade are required to have a written record on file at school of having received a 2nd dose of Meningococcal vaccine on or after their 16th birthday. Your child may or may not have already had this vaccine. The school does not have proof on file at this time. Please contact your health care provider for your child’s immunization record showing this vaccine date. If your child has not had a Meningococcal vaccine, you may obtain it from your health care provider or other community providers such as the Lorain County General Health District. You can schedule an appointment by calling 440-284-3206. Note: Ask your provider about additional vaccines that are recommended for children ages 11-19 such as 2nd dose of varicella (chicken pox), hepatitis A and human papillomavirus (HPV) vaccines. Proof must be provided to the school for the Meningococcal vaccine.
