Nursing Home Administrator (NHA) Application Checklist Information about the application process and how you will be contacted An Application Specialist will review your application based on the information you supply. If your application is incomplete, you will be contacted via the email address you provided. To monitor the status of your application, visit Online Services: apps.colorado.gov/dora/licensing/Default. Disclosure of Addresses: Consistent with Colorado law, all addresses and phone numbers on record with the Division of Professions and Occupations (DPO) are public record and must be provided to the public when requested. It is your responsibility to keep your address and contact information current in our system. Your email address is not open to public record, but must be provided at the time you register an account. If your email address is not current, it is possible you will not receive important information from DPO. You can change your address, email address and other information online by using Online Services: apps.colorado.gov/dora/licensing/Default. Application Expiration: Your application will be kept on file for one year from date of receipt in the Division of Professions and Occupations. Your file and all supporting documentation will be purged if you do not submit required documents and complete your application process in one year. You will need to resubmit a new application packet and fee after that time. If you submitted an application within the last year and were notified that you did not meet the requirements and you are submitting an update, you do not need to complete a new application or submit additional fees. License Expiration Grace Period for New Applicants: PLEASE BE ADVISED that if you are issued a license within 120 days of the upcoming renewal expiration date, you will be issued a license with the subsequent expiration date. For example, licenses issued between November 1, 2017 and February 28, 2018 will reflect an expiration date of February 28, 2019. Licenses issued prior to November 1, 2017 will reflect an expiration date of February 28, 2018, and must renew in the upcoming renewal period. 

All NHA licenses expire on the last day of February each year and must be renewed to continue practicing.

Printing your License upon Approval. You will be able to print your wallet card in its current status within 24 hours of licensure being granted. Login to your Online Services account: and select “Print Your License” at the bottom: www.colorado.gov/dora/licensing/Default. To obtain a NHA license, you must: Pay Required Fee: You must pay the $250 application processing fee with a credit card or electronic check to complete the application process. All fees are non-refundable and are subject to change. Be 21 years of Age: You must be at least 21 years of age to apply for NHA licensure.

Register for the National Examinations (if applicable): Once you have been approved to take the National examinations, the National staff will notify you to schedule an exam date. You can access the National NHA Exam application on NAB's website at www.nabweb.org/nha-exam. 

You must pay a fee for each National examination. Do NOT apply for the National examinations prior to being approved, as you'll be denied and subsequently charged for the exams. The National Association of Long Term Care Administrator Boards Exam Handbooks and references are available at www.nabweb.org/exam-references.

Register for the Colorado State Specific Nursing Home Administrators Examination (CO NHA Exam): Upon eligibility to take the CO NHA Exam, you will receive an e-mail from the test vendor ([email protected]). The Subject Line of the email will be Online Candidate Registration Exam: CO NHA. Check your E-Mail filers and “Spam” for this notice. The notice will contain instructions for scheduling the computer-administered CO NHA Exam. There is a separate test fee for the CO NHA Exam. 

A Candidate Handbook for the CO NHA Exam is posted on the Board of Examiners of Nursing Home Administrators website (www.colorado.gov/dora/NHA) | Find Applications & Forms | Nursing Home Administrator Applications and Forms | +Forms and Exam Information. The Handbook is entitled “Colorado State-Specific Nursing Home Administrators Exam Candidate Handbook”. Please download and review this resource before you schedule the CO NHA Exam.

Verify other Licenses: You will be asked to list ALL the AIT, NHA or other healthcare license(s) or registration(s) you hold or have held. You can provide scanned copy verification from the other state or jurisdiction website OR you can have verification emailed or mailed to DPO’s Office of Licensing – Attn: Nursing Home Administration Licensure ([email protected] OR 1560 Broadway, Suite 1350, Denver, CO 80202). The verification must indicate whether or not you have ever had disciplinary action taken against that license/registration. Affidavit of Eligibility: You will be asked during the online application to attest to and provide information that you are lawfully present in the United States or otherwise eligible to work here. You may be required to upload documentation of your lawful presence. Provide your social security number: As of January 1, 2009, a Social Security Number is required for all licensees. Exceptions are made for foreign nationals not physically present in the United States and for nonimmigrants in the United States on student visas who do not have a Social Security Number. These applicants must submit a signed Social Security Number Affidavit in lieu of a Social Security Number. Provide name change documentation (if applicable): If you already have another type of license in Colorado and your name has since changed, you’re presenting documentation for licensure with a previous name, or if any required documentation has a different name on it, you’ll be required to upload proof of your name change. Answer the screening questions: You will be asked a series of screening questions related to your criminal history and/or pending complaints filed against you in other jurisdictions (if applicable). This may require you to upload court documents or other material. Please review DPO’s information regarding the disclosure of criminal history contained at the end of this checklist. To obtain a NHA license by EXAMINATION, you must: Demonstrate your Education and/or Experience: You must meet ONE of the following education and/or experience requirements to qualify for NHA by Exam:  

You have successfully completed the Administrator in Training (AIT) program pursuant to Section 1239-107 of the Colorado Revised Statutes (C.R.S.); OR You hold a Bachelor's Degree or higher in ONE of the following: Public Health Administration, Health Administration, Health Care Management, Gerontology, or Nursing Home Administration; OR

You have a Master's degree in Management or Business Administration; OR You have a minimum of one year work experience in administration in a nursing home or hospital as required in Rule II.A.5 AND a Bachelor's degree in Business or Public Administration OR an Associate's degree or higher in a health care related field such as Nursing, Social Services, Sociology, Physical Therapy or Occupational Therapy from an organization nationally recognized by the U.S. Department of Education for Regional Institutional Accrediting.

Please be prepared to provide documentation of your education, experience or combination of both. If you are qualifying through experience, you must be able to upload the Verification of Experience form(s) at the time of application. To obtain a NHA license by ENDORSEMENT, you must: Currently hold an ACTIVE License or Registration: You must hold an ACTIVE NHA license or registration in another state or territory of the U.S. to qualify by endorsement. Request score transfer(s) from NAB: You must request a score transfer verifying you have passed the national examination OR another national examination administered by a nationally recognized testing entity in nursing home administration AND any other examination(s) required by any state(s) or jurisdiction(s) you have ever been licensed or registered. You may request your score transfer through NAB’s website: www.nabweb.org/nha-exam. Additionally, you must demonstrate ONE of the following: You have successfully completed the Administrator in Training (AIT) program pursuant to Section 12-39-107 of the Colorado Revised Statutes (C.R.S.); OR You've practiced as a NHA, taught in an accredited health care program, or served as a member of a nursing home survey or accreditation team for 12 consecutive months preceding submission of this online application; OR You've practiced as a NHA, taught in an accredited health care program, or served as a member of a nursing home survey or accreditation team for three of the five years immediately preceding the date of submission of this online application; OR Any other professional standard or measure of continued competency as determined by the Board, including the National examination. Please be prepared to provide documentation of ONE of the above. If you are qualifying through experience, you must be able to upload the Verification of Experience form(s) at the time of application. If you wish to qualify by another means, you must submit your request to the Board at the time of application.

Licensure and Criminal History

Thank you for your interest in becoming a licensed* professional within the Division of Professions and Occupations. Before you submit your application, please be aware of a few facts regarding criminal conduct, convictions, and disciplinary actions in other states. The mission of the Division of Professions and Occupations is “public protection through effective licensure and enforcement.” One way the Division safeguards consumers is by issuing licenses to fully qualified, competent, and ethical applicants. During the licensing process – and depending on the specific application – the Division may ask whether you have ever been disciplined in any state, arrested, charged, convicted, or pled guilty to a crime. An arrest, subsequent criminal conviction, or disciplinary action is not an automatic disqualification from licensure. Rather, the appropriate board or program will look at the facts surrounding the criminal conduct and disciplinary action in addressing your license application. You should know that licensure is a privilege, not a right. One thing you must do to obtain the privilege is to be complete and accurate in disclosing information on your application. Be sure to list all relevant complaints, disciplinary actions, arrests, charges, or convictions in response to the appropriate licensure questions. Failure to fully and accurately disclose requested criminal history information, alone, could constitute grounds for denial of your application or revocation of your license. When requested, you must include information regarding prior conduct. This remains the case when the conduct is seemingly unrelated to the activities of a profession, and when the conduct involves deferred sentences or judgments. Remember, even following licensure, you are still required to notify your professional licensing board or program about subsequent convictions and disciplinary actions in other states. Please be aware that the Division conducts audits of its licensing database against several criminal and national disciplinary databases. This allows the Division to verify the truthfulness of your application and track subsequent criminal and disciplinary conduct after initial licensure. Keep in mind, your license will not necessarily be revoked, or your application denied, if you have been disciplined, arrested, charged or convicted. But, you will most likely be denied or revoked if you fail to disclose requested information. *The word "license" is used as a general term. While most of the professions and occupations are licensed, others may be registered, certified, or listed. For precise terminology and requirements related to a profession or occupation, please consult the website of the appropriate board or program.
