5. Required Information. Contestants must enter the following personal information in order to become eligible to win the aforementioned prizes: (i) (ii)

Upon entering the Sweepstakes, contestants shall have the option to opt-in to receiving On Gossamer’s e-mail newsletter. 6. Effective date of entries. Entries are made online and will be effective on the day received. All entries become the exclusive property of On Gossamer and will not be acknowledged or returned. 7. Selection of winners. One (1) monthly winner will be determined randomly from all eligible entries received. Each contestant that is selected is deemed a potential winner pending confirmation and/or verification of the contestant’s eligibility and compliance with the rules, as determined by On Gossamer. 8. Notification of winners. Winners shall be informed via e-mail within a commercially reasonable time after their selection. All decisions are final and binding. Should the aforementioned notification e-mail be returned to On Gossamer as invalid or undeliverable for any reason whatsoever, two (2) more attempts shall be made to contact the winning contestant via e-mail. If the additional attempts to contact the winning contestant fail for any reason whatsoever, the contestant shall be disqualified and a new winner shall be selected. Winners must contact On Gossamer within seventy-two (72) hours from the time the notification e-mail is sent by On Gossamer to claim their prize. Failure to contact On Gossamer within seventy-two (72) hours will result in immediate disqualification of the selected contestant and a new winner will be selected. may occur in the collection and processing of entries; (ii) telephone, electronic, hardware or software program, network, internet or computer malfunctions, spam filters, failures or difficulties of any kind; (iii) failed, incomplete, garbled or delayed computer transmissions; (iv) any condition caused by events beyond the control of On Gossamer that may cause the Sweepstakes to be disrupted or corrupted; (v) any injuries losses or damages of any kind arising in connection with or as a result of the Sweepstakes, or from participation in the Sweepstakes; or (vi) any printing or typographical error in any material associated with the Sweepstakes. 12. Release/Indemnification. You agree to release and hold On Gossamer, its agents and representatives, its parent companies, employees, directors, officers, affiliates, subsidiaries, advertising, promotion and fulfillment agencies harmless from any and all losses, damages, rights, claims and actions of any kind in connection with the Sweepstakes, including without limitation, personal injury, death and property damage, and claims based on publicity rights, defamation or invasion of privacy. You understand that you are providing your personal information to On Gossamer. 13. Choice of law. All issues and questions concerning the construction, validity, interpretation and enforceability of these terms and conditions, or the rights and obligations of you and Sponsor in connection with the Sweepstakes, will be governed by, and construed in accordance with, the substantive laws (without reference to choice or conflict of laws) of the State of New York. In respect of any dispute regarding the subject matter hereof, the contestants hereby irrevocably consent and submit to in personam jurisdiction in the courts of New York, County of New York, including the United States courts located in and for the Southern District of New York, and to all proceedings in such courts. The parties hereby agree that such courts shall be the venue and exclusive and proper forum in which to adjudicate any case or controversy arising either, directly or indirectly, under or in connection with this Agreement and that they will not contest or challenge the jurisdiction or venue of these courts. 14.
