INFORMATION ABOUT COUNTY/CITY ONSITE FLU SHOT CLINICS DETAILS: Vendor: Health District of Northern Larimer County To register online, visit http://larimer.org/vaccination/ and follow the instructions. Each person needs a separate appointment. Only those aged 6 months and older can receive a vaccine at these onsite clinics. Vaccine is NOT preservative free. Vaccine is a “trivalent” vaccine; it has the H1N1 vaccine in it already. Members of the County or City Medical Insurance Plan should bring Insurance ID Card to appointment. Non-members must bring a method of payment (Visa, MasterCard, Discover, Cash, or Check). If possible, be prepared for your appointment by reading this Flu Vaccination Information Sheet and completing and bringing this Flu Vaccination Consent Form.

COST AT ONSITE FLU SHOT CLINICS: Members of the County’s or City’s Medical Insurance Plan: Free! Bring Insurance ID Card! City Employees NOT on the City Medical Insurance Plan: $10 All Others: $24 or less, depends on income & residency

VENDOR: Health District of Northern Larimer County REMINDER: Seasonal flu vaccine is considered a preventive benefit and is covered at 100 % for members of the County’s and City’s Medical Insurance Plan who receive a flu vaccine from an In-Network provider.

ALTERNATIVE OPTIONS: - County Employees and Family Members can also attend a Maxim flu shot clinic at Poudre School District. Free for those on County’ Medical Insurance with insurance card. $25 (cash or check) for non-members. See http://www.larimer.org/wellness/documents/PSD2010SEASONALFLUschedule.pdf for schedule. Vaccine at PSD flu shot clinics is not preservative free and is for ages 4 years and older. - County and City Employees and Family Members can also attend any 2010 community flu shot clinic sponsored by the Health District of Northern Larimer County. Must call 224-5209 for schedule and to set up appointment. Same cost applies as stated under COST AT ONSITE FLU SHOT CLINIC section above.
