Fomu hii ni badala ya fomu nyingine zote zinazotumika kupima utendaji kazi katika Utumishi wa Umma. Aidha inakusudiwa kukidhi utekelezaji wa shabaha na malengo ya menejimenti katika utumishi wa umma na kujenga uhusiano mzuri wa utendaji kazi kati ya viongozi na wanaowaongoza. DONDOO ZA NAMNA YA KUKAMILISHA FOMU: 1.
Fomu hii ijazwe na watumishi wote walioko katika Utumishi wa Umma. Kwa Maofisa Wakuu (Principal Officers) na kuendelea baada ya kukamilisha fomu hii nakala halisi iwasilishwe kwa Katibu Mkuu (Utumishi), nakala ya pili ipelekwe kwa Mtendaji Mkuu wa Taasisi/Asasi husika, na nakala ya tatu apewe mtumishi anayehusika. Maofisa wengine wote kuanzia Maafisa Waandamizi (Senior Officers) kwenda chini nakala halisi iwasilishwe kwa Mtendaji Mkuu wa Taasisi/Asasi husika, nakala ya pili ipelekwe kwa Wizara Mama ya kada husika na nakala ya tatu apewe mtumishi anayehusika.

Pale panapohusika kila kisanduku hapa chini kikamilishwe ama kwa herufi kubwa au tarakimu moja.

Malengo yatakayohusu mtumishi yatokane na mpango wa kazi wa Taasisi (Strategic Plan) ukiainisha mpango wa utekelezaji kazi wa mwaka na ratiba ya kazi ambayo inatarajiwa kutekelezwa katika mwaka huo.

Sehemu ya 2, ya 3 na ya 4 ya Fomu hii ikamilishwe na mtumishi anayepimwa kwa kushirikiana na Msimamizi wake wa kazi na Sehemu ya 5 na ya 6 zikamilishwe akiwepo Msikilizaji kama ni muhimu.

Zingatia kwamba alama 1 ya utendaji wa kazi ni ya watendaji waliotekeleza malengo yote na ya ziada kwa ufanisi na alama 5 ni ya watendaji wasioridhisha na wasiotimiza malengo. Taarifa za hawa wote zifikishwe kwa mwajiri wa Taasisi/Asasi husika.
