Please fill in the blanks with the child and parent information. Make two copies and send one to the school and the other to Chicago Public Schools-CEO Arne Duncan at 125 S. Additionally, please consider this letter a formal written request for a due process hearing, as provided for in the Illinois School Code, on behalf of my child, [____________Student Name _________] so that he may be allowed to re-enroll in school. I believe my child can meet the necessary graduation requirements prior to his/her 21 st birthday, and is therefore entitled to receive a public education as provided for under Illinois State law.
