TO: Parents of 5th and 6th Grade Boys and Girls FROM: Greg Goan, Athletic & Activities Coordinator RE: Start of Intramural Dodgeball DATE: April 3, 2017 The purpose of this note is to inform you that Dodgeball will start for 5th and 6th grade boys and girls on Tuesday, April 25th, and run for 12 sessions through June 1st. Dodgeball will be held on Tuesdays & Thursdays from 2:45 – 4:00 p.m. Students should be picked up promptly at 4:00 p.m. or plan to take the late bus. Please sign and return the attached permission form by Monday, April 21st. Participation is limited to 40 so 1st come, 1st serve! Westbrook School Department Student Insurance Permission for Student to Participate Policy JLA The Westbrook School Committee provides minimum insurance coverage for students participating in Westbrook School Department sponsored interscholastic sports and/or activities. Parents are responsible and encouraged to purchase school child accident insurance which covers their child during school day activities, intramural activities, and accidents which may occur on school property. Notification explaining school child accident insurance will be provided at the beginning of the school year. This insurance may be purchased throughout the school year. Prior to participation in any school sponsored activity, permission slips will be filled out and signed by parents. Nothing in this policy shall be construed as financial responsibility by the Westbrook School Department for students injured in any school activity when reasonable precautions have been taken and proper supervision provided.
