I would like to declare my intent to participate in the College Credit Plus program. I understand that signing this form does not require that I participate during the coming school year and I may decide not to participate without consequence. I also understand that it is my responsibility to notify my school if I do not gain admission to my selected institution of higher education or choose not to participate for some other reason. I certify that I have received counseling about the College Credit Plus program concerning the rules and regulations for both my school and the college, and that I understand my responsibilities, the benefits and possible risks of participating in the College Credit Plus program. I also understand the following: •
For any courses offered within the district, I authorize the district to act as proxy and arrange or alter my enrollment as necessary with the college in my stead and request this form be accepted for adding or dropping courses.

Completion of this consent form does not guarantee admission to a specific program or course and that classes may be cancelled. Students must meet course placement and/or prerequisite for courses requested.

Courses will become a part of student’s permanent academic record and will be included on the high school transcript and calculated in the high school grade point average, and may impact the student’s ability to graduate high school.

If student fails a course or withdraws after the college’s refund deadline, the district may bill the family for the cost of attendance. Books issued to the student by the college are property of the district and must be returned. Nonpayment of any fees for a lack of course completion or materials left unreturned may impact graduation.

Students may be required to interact with classmates on group work and projects inside and outside of the classroom. The subject matter of courses may include mature themes and materials and will not be modified for students participating through College Credit Plus. This includes films rated “R” or books with mature content.

Students with documented disabilities are responsible for requesting a review of accommodations through the college’s Office of Disability Services in order to receive services deemed appropriate for the student.

While courses funded through College Credit Plus may be taught on district property by credentialed district personnel, such courses are considered a post-secondary environment. In compliance with the federal Family Educational Rights and Privacy Act (FERPA), students (including minors) have all rights transferred to them. Parents should not expect instructors to share information about student progress, attendance, assignments, etc.

Information that is considered an institutional record exchange (grades/attendance) will be posted on Infinite Campus.

Students must request official transcripts from the district’s college partner for credit to be formally transferred.

The cost of books and course materials is borne by the district. Books purchased by students or their families independently and without written permission in advance from the district treasurer are considered the student’s personal property and not subject to reimbursement through the program.
