Please bring the following items with you for Pre-School registration:

Child’s Social Security Card

Proof of Child’s Residence (example: utility bill mailed to your home address)



Proof of Household income (showing amount received)



Child’s Updated Immunization Records



Name, Address, and Phone Numbers of three (3) contact persons



Name, Address, and Phone numbers of Child’s Doctor and Dentist
