Thank you for requesting the YASD Prize Wheel. The following are a couple of simple guidelines that we have set up to ensure that the Prize Wheel continues to be useful for all members. 1. Priority will be given to YASD Members 2. The borrowing librarian is responsible for pick-up and return to the location set by the current YASD President. The Prize Wheel cannot be delivered in the SCLS van. 3. The wheel must be used for Young Adult Programming purposes. 4. There is a TWO week loan period for the wheel, unless no other library has requested the wheel, in which case you will have THREE weeks. 5. Requesting library is responsible for any damages incurred to the prize wheel during the length of time used at the library. Please fill out this form below and return to the current YASD President.
