Boscobel Area Schools 1110 Park Street Boscobel, WI 53805 Request for Proposals The School District of Boscobel is seeking proposals for the following work:

Replace Sewer Line from Main Lobby Bathrooms to City Connection on Brindley Street from Boscobel High School 300 Brindley Street Boscobel, WI 53805 Scope of Services: 

  

Work includes replacing and installing sewer lines from the main lobby bathrooms to the city connection on Brindley Street. Removal of part of a classroom floor will be necessary in an area between the tunnel and a point beyond the school wall. Trenching will include removal of a small concrete area and grassy areas. Interested bidders should contact Greg Bell at 608-375-4164 ext. 2511 to set up an appointment to discuss expected work to be performed. All required permits will be the responsibility of Boscobel Area Schools Work to begin after the last day of classes in June and must be completed prior to the start of school in August 2017. The successful bidder will be required to meet with staff from the School District of Boscobel, City of Boscobel and inspectors to ensure the timely coordination and completion of this project.

Insurance: Prior to beginning replacement project, the School District of Boscobel requires that any awarded contractor providing materials, equipment or services to the School, must provide the

school with a certificate of insurance naming the School District of Boscobel as an additional insured, for the following:  

   

Commercial Umbrella Insurance in the amount not less than $1,000,000. General liability (including completed operations coverage) in the amounts of $1,000,000 (combined single limit) Bodily Injury/Property Damage coverage per occurrence, and $2,000,000 general aggregate coverage and include a waiver of subrogation on behalf of the School District of Boscobel. Automobile Liability in the amount of $1,000,000 (combined single limit), Property Damage and Bodily Injury Coverage. Professional Liability, in an amount not less than $500,000.00 per occurrence and $2,000,000.00 aggregate. Worker’s Compensation as defined in the Wisconsin General Statutes and include a waiver of subrogation on behalf of the School District of Boscobel. Any subcontractor to a contracted firm shall be likewise covered, and shall furnish certificates of coverage acceptable to the School before starting work.

Invoicing and Payment: Invoices shall be paid promptly by the School unless any items thereon are questioned, in which case payment will be withheld pending verification of amount claimed and the validity of the claim. Standard payment terms are Net 30 Days from receipt of properly executed invoice(s). If your firm submits a proposal that includes payment schedules based on the completion of designated phases, those stages must be clearly outlined in your proposal. The School District cannot make payments for “Execution of contract” (payments due upon contract signing). General Instructions: Proposals are to be:        

Submitted in a sealed envelope with the bidder’s name and address displayed on the front of the envelope. Proposals received later than the time and date specified will not be considered Amendments to or withdrawal of proposals received later than the time and date set for the opening will not be considered. Bidders or their representatives may be present at the bid opening. The School District of Boscobel Area Schools may require further information and references on any individual or company placing a bid prior to the awarding of a bid. The School District of Boscobel Area Schools reserves the right to amend and/or cancel the bid invitation prior to the time and date of the bid opening. The School District of Boscobel Area Schools reserves the right to accept or reject any or all bids and to accept the bid they deem to be most advantageous to the district. The School District of Boscobel Area Schools reserves the right to correct an award erroneously made as a result of a clerical error on the part of Boscobel Area Schools.

All sealed proposals must be made in accordance with the specifications supplied herein and are due at the following address no later than 4:00 pm on Monday, June 12, 2017. Bids must be
