1. Excavation(Physical Hazard):Workers are seen in a ditch working on drainage facility in the construction area about eight feet in depth and observed that no shoring or other means of protection which may lead to cave- ins and lead death to employees and/ or serious injury but not limited to limbs fracture, head injuries and no signage of authorized personnel only which may lead to slip and fall and injuries from cuts, bruises or bone fractures for workers. 2. Physical and Chemical Hazard: A painter is seen working on the 7thfloor level at 40 feet height painting outside of the building in the construction area of Phase-I using boatswain’s chair and brush, subject to risk of falling from height potentially as he had neither safety belt and nor safety harness tied to its body to protect from fall could cause loss of his life or serious injury such as fractureof upper/lower limbs and/or head injury, Back pain, Chemical hazard and ergonomic hazard.

Short Term Measure:

3.Fall from Height Hazard: Workers using scaffold at 45feet high for construction had not been mounted well on the even surface and had no guard rail, outerriggers broken and not replaced, not enough braces and toe boards have large gaps on the work platform which all could lead to falling

Collision Hazard (Physical): Forklift trucks at the warehouse in the construction site are observed that rounding a blind corner into a loading area without stopping can lead to collision with workers, collision with stationary objects and collision with other vehicles which can cause from death to serious personal injury by falling objects and also lead to property damage by topple and/or collision. 5.Hot Work (Fire Hazard): Welding rods work at construction site are happening near the LPG gas storage area which could lead to massive fire or explosion wherein other flammable objects such as timber woods could lead to massive casualties, environmental pollution and property damage.

Short Term Measure: . Provision of side mirrors on all blind corners where driver can foresee the visibility of vehicles and workers movements. Long Term Measures: .Provision of banks men at the blind corners. .Adequate training to all forklift drivers on the safety practices of driving forklifts on the blind corners and spots or speed bumps at the blind corners.

6. Electrical Hazard: Excavation area at Phase-III of construction sitehad high voltage (10000v)power supply of electricity for the whole construction area had no barriers or danger signage and workers exposed may lead to electrocution ,electric burns and secondary effects such as thrown off to the excavated area lead to serious injury from head injury to death.

Short Term Measure: Danger voltage signage needs to be placed.

7. Physical Hazard: Excavation area at the construction site of 8ft depth has not been protected by barriers which may lead to falling people by slip, topple of excavator and falling of object can lead from death to serious injury but not limited to bone fractures, or limbs disabilities and head injuries.

Short Term Measure: Stop the excavation work in progress.

8.Physical Hazard: Drilling machine used at the construction site for concrete coring produces high level of noise and vibration which can lead the users from temporary ringing to permanent ringing of ears(Tinnitus),

Short term Measure:. Use of appropriate PPE such as gloves to protect moderately from vibration and ear muff to protect from noise.

10. Ergonomic Hazard: Manual handling of carrying cement and bricks construction materials by one of the workers using wheel barrow weigh approximately 150kgwhich can lead to back pain, rupture of intervertebral discs, overloaded muscle can tear.

Short Term Measure: Instruct to carry only safe working load limit which is 40Kgs.

11. Physical Hazard: Concrete mixer workers have been exposed to hot weather condition of approximately over 43c could lead to dehydration, heat stress /strokes.

Short term Measure: Frequent breaks and job rotation methods by instructions by properly preparing working schedule for staffs.

12.Slip (Physical )Hazard: Spillage of oil from one of the forklifts on the worker’s pathway can cause slip and fall and may lead from serious injury like bone fracture to lower injury such cuts and bruises.

Short Term Measure: Clean the oil spill by placing wet floor sign.

13. Psychological Hazard: Observed that one of the workers at the construction site were shouting and yelling at the other co-worker to do something which can lead to stress and demotivation in front of other workers.( Behavioral issue)

Short term measure: Consultation with both employees about the situation and reason for yelling at the work place.

14.Physical Hazard: Poor lighting at the warehouse where workers needs to get construction materials to the site might lead to slip, trip and fall cause serious injury like head cuts, bone fracture, Limbs disorders etc.

Short Term Measure: Adequate lighting need to be provided where workers have more access to materials on daily basis

15. Chemical Hazard: Poor ventilation at the cement storage area where workers need access to get cement to the construction site and prolonged exposure can lead to respiratory problems, headache, irritation to eyes.

Short Term Measure: Instruct the worker not to enter cement storage area.

16. Physical Hazard: Emergency fire exit door has been obstructed with wooden boxes which can lead to trip and fall hazard may cause death of many casualties due to major fire and lead to serious injury from head to fracture of bones

Short Term Measure: Remove the obstruction from the emergency fire exit door.

17. Trip Hazard: Trailing of cables at the back office work area where marketing employees were doing the booking of apartments can lead to trip and fall to both employees and guests could lead to bone fracture, head injury.

Short Term Measure: Immediately remove all cables from the walking areas and floor and mount it through ceiling. Long term measure :Showing training videos for workers at risk and hidden hazards present in accident of trailing cables,

18. Ergonomic Hazard: Sales &Marketing department employees have a poor working posture looking at the computer due to nonadjustable chairs and computer monitors can lead to musculoskeletal disorder, eye strain, fatigue and back pain.

Short Term Measure: frequent breaks and job rotation by having tailor made good working schedule.

19.Biological Hazard: Been observed that there is a pond located near right side of the construction area where all drainage water of the village are stagnated can cause biological effect of inhaling and/or absorption of harmful bacteria and viruses may lead to contagious diseases or Dermatitis,Legionellosis.

Short Term Measure: Water treatment with chlorine (eg.chlorination) to kill the bacteria.

Long term measure : changing the cement storage area to adequate ventilated area by installing Dilution Ventilation system.

Long term measure: Reiterate employees about the importance of fire evacuation emergency procedures in place by training and to maintain good housekeeping.

Long term measure :Purchasing adjustable chairs and monitors where workers can adjust to adequate level

20. Mechanical Hazard:Electric disc cutter machine (110V) for stone cutting had no guards and could lead to potentially high risk for entanglement as he is wearing loose clothing, and cutting and/or abrasion as he is not using any PPE.

Short Term Measure: Instruct worker to use PPE Gloves and not to wear loose clothing to prevent entanglement.

21. Cafeteria for construction workers and management has been found in tidy and hygienic condition with healthy menu which demonstrates the commitment of the management.

Appreciate the management and cafeteria staff for designing healthy menu and housekeeping of entire cafeteria.

Long Term Measure:Provide training, instruction and demonstrating videos to the workers on procedures of operating electric stone cutters and its importance.

22. Locker facility allocated for each and every construction section employees separately according to their needs and nearest to the place of work for easy access.

Demonstrates the management commitment towards health and safety of their employees.

23. First aid provision at each construction sections and first aid training for all employees given during orientation mandatorily.

It demonstrates the importance of first aid provision and how it can be utilized well when unplanned event occurs at work site.

24. Drinking water facility at each floor level for employees with hot and cold options according to the worker needs.

Promotes good working atmosphere for employees and ability to work with morale.

25. Toilets have been maintained clean and tidy with all provision to the workers such as hand wash liquid soaps, toilet rolls (big and small), air freshener and air-conditioned.

Good commitment from the management towards the hygienic facility of toilets with provisions.

Introduction including overview of area inspected and activities taking place:

I am Paul prepared this report follows the health and safety inspection of one of the biggest construction projects of Shoba Developers a leading real estate company in India and abroad has the upcoming project as Shoba Terrain at Devanahalli Bangalore which comprises of three phases, warehouse and administrative building which had both indoor and outdoor activities and its spread over 20 acres located at Devanahalli, Bangalore north which is close to our Bangalore International airport. The inspection took place on 28 march 2014 between 1000 and 1300hrs. Phase-I and II each comprises of 12 floors and have 460 apartments where the indoor activities such as painting ,drilling , plumbing, and electrical connection are happening and it is almost in the handover stage. Phase-III is at the initial stage of construction where the main outdoor activities such as excavation, building concrete, concrete mixing and welding works are happening which comprises of 230 apartments&12floor levels and expected to complete by March-2014. Warehouse where indoor activities such as safe stacking and storing of construction materials such as cement ( separate storage area) in the warehouse and welding rods, bricks, woods . Workers in administrative building are carrying out registration activities of apartments & other administrative indoor activities such as printing, copying and emails. Management and administrators of about 20 employees and 400 construction workers were present while this inspection was conducted. The aim and reason for this inspection report to highlight the areas of concern about existing health and safety hazard and to reduce the accidental statistics of previous year, also to measure the standard of health and safety management system maintained and to check whether it meets the legal standards.

Executive Summary

In 2013, there have been 12 accidents recorded on this construction site, out of which 7 minor from cuts and bruises and 5major injury of bone fracture, head injury due to worker fell from height .This inspection observed some major areas of health and safety concern on both outdoor and indoor activities carrying out by workers which can put them under risk but not limited to death, serious injury and property damage. Major findings and its recommendations:  Workers are at high risk of cave-in at the ditch where drainage pipeline work is happening and construction of hydraulic shoring could prevent from death to serious injury. th  Painter is working at height of 40feet( 7 floor) on boatswain’s chair with neither safety belt and nor harness which might lead to fall from height cause serious injury to death and cherry picker with electric foam painter has been recommended.  Scaffold not erected on the flat surface, no guard rails braces are not well fitted and some missing, toe boards have huge gaps can lead to scaffold collapse , people and object fall from height and purchasing new scaffold is highly recommended.  Forklifts are not stopping at the blind corners and can lead to collision with other vehicle & workers and topple forklift if the load is heavy may cause from death to serious injury and property damage , using banks men and side mirror at the blind spots are highly recommended.  Rods have been welded close to the LPG storage area which can lead to explosion and fire and cause from several personnel death to serious injury and property damage, a separate covered welding place is highly recommended to prevent from this risk.  High power electric supply near excavation area needs to be covered by lock out-tag out isolation system.

Inspection also observed good welfare facilities to workers from clean toilets with liquid hand soap, drinking water at each floor levels, first aid provisions and lockers, however requesting your earliest attention on the hazards which outlined above to reduce the risk to acceptable level which subsequently enable to decrease the accident records for this year.

Main findings and recommendations of the inspection:

1.Cave- In/Trenching : Workers are in a ditch of about 8 feet in depth doing drainage pipeline work and found that there is no protection from cave- in and slip and fall into the ditch as well as there is no protection and signage stating authorised personnel only or prohibited area which breaches the ILO legislation of Convention C167-Safety and Health in construction convention, 1988(No.167). Recommendation: Hydraulic Shoring of trench would prevent workers to fall from height by slip or soil to cave in and would cost approximately $200 and it is easier to fix by one person and can be used for new upcoming projects as well. 2. Painter working at height:. th Painter was working at the 7 floor level of about 40feet high painting outside the building Phase-I using boatswain’s chair and brush, may fall from height as he had no safety belt and harness and may exposed to inhalation of chemical substances in paint which may lead to suffocation and manual handling at height using hands lead to muscle pain and stress and its breaching the legislation of ILOC155 Occupational Health and safety convention 1981 and Work at height regulation-2005(WAHR).

Recommendation: th

Use cherry picker which is available with us for the painter to access at 5 floor level for painting and purchase of one electric paint sprayer with face piece respirator would prevent the chemical exposure and fall from height and also avoid much manual handling. Sprayer and respirator would cost around $120.

3.Scaffold collapse, Falling of people and objects: Scaffold being used by the workers at construction site level of 45 feet high is not mounted on the even surface , out riggers were broken and not being replaced, toe board have huge gaps and some braces been missing and others are not fitted properly which may lead to collapse of scaffold, falling of people and object from height can lead to fatal , serious injury from head injury to muscle stress and pain breaching the international legislation of Convention C167-Safety and Health in construction convention ,1988(No.167) and Health and safety at work 1974(HASWA). Recommendation: Purchase of new scaffold which would save many people lives and prevents falling of objects and collapsing of scaffold which would cost around $350. 4. Collision with workers, vehicles and Property damage: Forklift trucks at the warehouse in the construction area are observed that rounding a blind corner into a loading area without stopping can lead to collision with workers, collision with stationary objects and/or vehicles which can cause from death to serious personal injury by falling objects on victims and also lead to property damage by topple of forklift which breachesILO C155 Occupational Health and safety convention 1981 and Health and safety at work 1974(HASWA). Recommendation: Provision of side mirror on the blind corners where driver can foresee the movement of people and vehicles which ultimately prevent collision and it would cost $40 and also provision of banksmen for the blind spots and corners. 5. Fire hazard at the welding area: Welders are welding the rods which is closer to the LPG gas storage area and sparks while welding can contact with the LPG gas lead to massive fire/explosion cause property damage and mass casualties in the workplace which breaches the international legislation of Convention C167-Safety and Health in construction convention, 1988(No.167) and Fire protection and prevention Act 1997. Recommendation: Provide a separate covered welding area for the whole construction site where all welding works can take place may cost $550 which eventually prevents mass casualties and property damage. 6. Electrical Issue: Main electric power supply for the entire construction site located near the excavation area is unprotected and no signage of danger being placed can lead to electrocution, burns, death and secondary effect like fall from height due to throw off can cause serious injury of bone fractures and head injury which breaches theILO legislation of C155 Occupational Health and safety convention 1981 and Electricity at work regulations 1989(EAWR). Recommendation: Barricade the high danger electricity power supply area by isolating( lock out –tag out system) and would cost $200 can prevent electrocution, burns, even death and secondary deaths such as fall from height by being victim throw off due to electrocution and fall into excavated area cause head injury or serious personal injury.

Conclusion: Firstly, I would like to take this opportunity to thank Shoba management &each and every worker at site for their outstanding cooperation while conducting inspection without which I could have not done this observation about health and safety practices at construction site. Few major findings needs immediate attention towards the safety of workers from the management such as “ Cave-in, Fall from height( People and objects), Electricity & Fire hazard, collision with vehicle and pedestrians at warehouse by forklifts which can be controlled in near future by incorporating suitable recommendations .Other safety concerns such as manual handling, vibration, noise, stress, biological effect, chemical exposure, heat stress, poor lighting and ventilation can also be controlled with suitable preventive control measures which also recommended on the observation sheet. The workforces were seemed to be very happy when I inquired about their well-being within the facility and they echoed there is no company can compete the welfare facilities such as locker, first aid provisions, drinking water, healthy menu at cafeteria and cleaner toilet facilities and also told what else one would expect? Which brings enthusiastic myself when I hear them which clearly demonstrates the brigade management commitment towards health and safety of their workers. Lastly, I would request to please take necessary actions on the suitable recommendation outlined for existing major hazards and I look forward for your support to further enhance our business reputation by protecting our workers &property and meeting the obligation of national and international legislations and for our next ISO certification due in 2 months for renewal without any hindrance.
