1. Deep excavator hole size of 2x2x3 meter wide without any shoring protection and has the high hazard of people/material can fall and has the potential for fatality, serious personal injury such as bone fracture &head injury and also property damage(Physical hazard)

Tower crane at site did not have an operational/safety certification nor did the crane operator have his operator license on hand/demand..Lifting accessories were not inspected before and after the operation. Workers have been exposed to hot weather and working for long time without adequate breaks provided leading to faint, dehydration and hyperthermia.(Physical Hazard)

Short Term Measure: Appoint interim banks man to guide operator.

9. Diesel generator has found to be leaking and there are no spill trays to collect them hence fumes can lead to possible fire and /or explosion having a potential damage to property and also loss of many lives (Chemical Hazard).

Short Term Measure: Buy a spill tray and fix where observed leakage. Long Term Action: Identify the cause for the leakage and rectify and impart training on the fire and /or explosion. .

10. Workers were manually lifting cement bags from the warehouse and also found to be not using PPE while carrying out subject to ligament damage(Ergonomic Hazard)

15.Trailing of cables at the drilling area where workers found to be walking around that area could lead to trip hazard and have the potential for head injury, bone fracture.(Physical Hazard)

Ergonomic Hazard: Administration department employees have a poor working posture looking at the computer in the remote office area at construction site due to non- adjustable chairs and computer monitors can lead to musculoskeletal disorder, eye strain, fatigue and back pain. 17. Biological Hazard: Been observed that there is a pool of water stagnated located near right side of the construction area can cause biological effect of inhaling and/or absorption of harmful bacteria and viruses may lead to contagious diseases or ,Legionellosis, Hepatitis A, B. 18. First aid provision at each construction sections.

Short Term Action: Frequent breaks and job rotation by having tailor made good working schedule.

Long term measure :Purchasing adjustable chairs and monitors where workers can adjust to adequate level 3 days. Short Term Action: Water treatment with chlorine to kills germs, bacteria and virus. Long term Action: Purchase of biocides to kill germs completely and proper housekeeping with adequate supervision.

19. Wholesome drinking water at all construction areas.

Potable drinking water to workers would help to hydrate themselves and employer mention with the signage board stating “Safe Condition” at each drinking water facility areas.

1 day. 20. . Mechanical Hazard: Electric disc cutter machine (110V) for cutting stones in the construction where it had no guards and could lead to potentially high risk for entanglement as he is wearing loose clothing, and cutting and/or abrasion as he is not using any PPE .

Short Term Measure: Enforce worker to wear gloves PPE to minimize the abrasion. Long Term Measure: Training for all place workers on using the electric disc machine using hazard without using guard and replace the missing guard.

21. Employees are at high risk of lung diseases from cement dust around the concrete mixer. There is no protection measures have been taken for employees such as no PPE,RPE and hard hats can lead to asphyxiation, scar lung tissue and also irritant to skin, eyes(Chemical Hazard).

Short Term Measure: Enforce the worker to use PPE/RPE such as hard hat and gloves, RPE is single face piece mask respirator to protect from inhalation hazard. Long Term Measure: High impact training associated with the use of cement/concrete mixer.

22.One of the workers were yelling at another in the construction area can cause stress and have the psychological effect as poor self esteem and depression(Psychological Hazard).

Short Term Measure: Consultation with both the employees to identify the cause.

23.Gas cutting work is ongoing nearby fuel storage area(Diesel). There is no fire protection against burn flying objects and fire extinguisher around that area. Risk of the property damage/fire incident/loss of lives)(Chemical &Physical Hazard).

Short Term Measure: Interim appointment of Fire Watcher.

24. Welding work found to be happening around fuel storage area, hazard of fire/explosion as the spark from welding operation would do and risk of property damage, loss of lives, burn injury(Physical Hazard).

Short Term Measure: Appoint fire watcher in that area.

Long Term Measure: Buy pre-fabricated welding pipes instead carrying out welding operations.

Introduction including overview of area inspected and activities taking place

The report follows a health and safety inspection of the construction, administration office and welfare facilities at Prestige Group, Flyover construction, Surat, India where company involves construction projects of apartments and also road projects like flyover and pavements. The inspection took place on 30 March 2014 between 0900 and 1600. The scope of this inspection is to highlight areas of concern because there have been serious accidents and near- misses for the past month. Around 300 workers at site and using hand held tools such as hammer, chisel, axe and power tools such as drilling machine, portable electric disk cutter, crane and cement mixer etc.. Inspection has been conducted based on the visual observation and also in interviewing workforce and checking documents such as tool box talk, investigation report of previous accident that has happened in this workplace for the past months ,Permit to work , safe system of work documents to overview the general health and safety culture and commitment towards health and safety of the organisation. Site has ongoing work of stacking construction materials using crane, excavator to dig up the soil for the foundation ,welding operation ,gas cutting operation and concrete mixing operation.

Executive Summary At the completion of this inspection, I have concluded with recommendation which includes the time, effort, money and control measures to mitigate/eliminate the health and safety issues observed through are pointed out .Implementing on these control measures not only benefit the company business image also boost employee’s morale and avoid any legal implication from the external authorities, Direct and indirect cost associated for the company following an accident. Key issues were found in the construction area activities are highlighted below.    

Excavation for the foundation of flyover with no shoring/trenching could lead to people/material may fall. Materials were stacked too much height and had the potential for falling from height on to the workers lead to head injury, fatality as some objects observed to be sharp ones. Tipper Lorries and excavators are reversed without the help of banks man could lead to accident with fixed objects/pedestrians. Tower crane operator without any license and certification when spot checked and also no inspection being done on the lifting materials and only visual contact with operator and spotter no other communication device had been used for potential of blind spots for crane operation.

A quick analysis of the cost shown in the recommendation can help to identify the cost implication to overcome the key areas which can in turn help to protect our workers, property and more importantly our ISO certification renewal and public image on our business.

Main findings of the inspection The key areas that have been identified are listed below: *Observation#1: Deep excavation were found for the foundation of flyover without any protection measures from the workers/materials to prevent fall and also soil heaped around the excavation area can cave-in and has the potential for workers can caught in lead to asphyxiation, death and also head injury, cuts, bruises when workers fall into excavation and it is breaching ILO legislation of Convention C167Safety and Health in construction convention, 1988(No.167). Recommendation: Trenching the excavation area would completely eliminate the soil from cave in and also warning signage around would caution the workers to understand the importance of signage and helps to protect themselves from falling /also falling objects when workers working inside. For trenching would cost approximately $250 and for purchase of 2 new warning signage cost around $50.

*Observation#2: Construction materials such as iron rods ,bricks, cement bags were stacked too high where workers and publics are walking nearby and it has the potential to fall off from height , workers/public may get crushed and result in serious personal injury not limited to bone fracture,cuts,bruises etc... Which is against ILO-C155 Occupational Health and safety convention 1981 and Work at height regulation2005(WAHR). Recommendation:

High-Impact aversion therapy training for all workers on safe stacking to completely eliminate this issue and also barricading the area helps to prevent the pedestrians movement safe which both could cost approximately USD250. Observation#3: Tipping operations being carried without the help of spotter where lot of workers were found to be moving around and also no announcement being made to clear off the area that could alert the workers to move away before the tipping operation started has the potential for workers even get crushed when the load fall on them resulting in serious personal injury,fatality,head injury etc. which breaches the legislation of ILO -C177- Home work convention ,1966(No.177) Recommendation: Interim provision of banks man allocated before tipping operation could start minimize the issue, to completely eliminate this issue would be announcing before tipping operation starts and recruit a spotter and train to master on signalling to the operator and ensure no one is around during tipping operation which would cost approximately USD400. Observation#4: Tower crane operator where found to be no paper work of his license and certification that supposed to be carried in the crane machine. Also no inspection/maintenance record where in place for crane and its slings where it all could lead to serious property damage and fatality which breaches ILOR164-Occupation health and safety recommendation,1981(No.164) Recommendation: Enforcing strict rules all necessary paperwork should be in place in the crane such as operator’s license and certification for crane and also inspection/maintenance records carried on weight lifting equipments during operation .

Observation#5: Excavator were reversed without the help of banks man and there are plenty of other workers working around the excavation area and also no signage in place can lead to excavator may fall into the excavation can cause property damage and also when it collide with pedestrians can leads to fatality by getting crushed which breaches ILO-R175- Safety and health in Construction Recommendation, 1988(No.175) Recommendation: Appoint interim spotter to minimize the consequence and to completely eliminate recruit and train spotter which would cost approximately USD450.

Conclusions At the outset, I would like to thank the management for giving me an opportunity to inspect on the place where there is heavy mounted traffic and it found to be challenging with one of the biggest construction of flyover project at Surat, India. Also would like to extend my thanks to each and every workforce who helped me throughout this inspection, by understanding the importance of why I am being here as it pertains to their safety instead that the inspector came to point out the issues out there and hence misbehaving like not providing sufficient information when asked/look at me like a police. Workforce seems to be happy with the sufficient welfare facilities and adequate workload on them with the shift basis, however there found to be still grey areas where management should focus on to rectify them, such as    

Excavation without protection could lead to falling of material/worker has the consequence of serious personal injury and fatality, constructing a trench with adequate warning signage would eliminate this hazard. Construction materials were stacked too high has the potential to fall from height on to workers can have significant head injury ,bone fracture etc. Reducing the height and training would help to mitigate this issue. Tipper and Excavators were reversed/operated without the help the banks man can lead to striking of pedestrians around that area, to rectify this issue an interim spotter should be identified to mitigate and to completely eliminate recruit and train the spotter. Crane operation being carried out without any inspection record and also no paper work on board such as crane operator’s licence and certification which could lead to cancelling the license for that vehicle to operate and any accident may found to be breaching the statutory law,to rectify ensure inspection carried out on schedule and necessary paper work on board all the times.

Above issues were spotted as major which highlights the concern of serious moral, legal and economical problems which in turn could damage the public image and reputation on us. Suitable recommendation suggested on the spotted issue which I’m bringing to your attention through this report for implementation to rectify it without further delay.
