WHAT is Colorado SIM & WHY is it important?
The Colorado State Innovation Model (SIM) touches nearly every aspect of our health system, setting the stage for a sweeping transformation that will help us accelerate our progress toward the Triple Aim of lower costs, better care and improved population health. Central to transforming the Colorado health system is the integration of behavioral health and primary care. There is strong evidence that treating physical health, mental health and substance use disorders together will help reduce the burden of chronic health issues.

Participation Qualifications: - Colorado primary care practice - Leadership commitment to moving toward behavioral health integration - Electronic health record (EHR) in place - Experience with practice transformation, including team based care, quality improvement, population management and using data for improvement
Important Dates (for Cohort 1) : - Practice RFA OPENS: September 15, 2015 - Practice RFA CLOSES: October 26, 2015 - Selected Practices NOTIFIED by: December 25, 2015 - Cohort 1 Practices KICK-OFF: February 1, 2016
Practice Improvement: Support to implement models of care that improve the quality of care and emerging frameworks for payment as well as practice facilitator and clinical HIT advisor provision. Transformation Fund: Payments to help offset some costs, including attending learning sessions, collecting measures and participating in evaluation. Payments from Health Plans: Colorado payers are working to expand value-based payments to practices engaged in transformation efforts. MOC and CME: Maintenance of Certification (MOC) and Continuing Medical Education (CME) credits available.
