Work Team: Administrative Services Building: RESC Work Team Leader: Brittany Parkhurst Results for Goal #1: Goal 1 four our group was to generate and distribute a "who to know, where to go" document in order to provide key administrative services contact information. This goal was set to improve upon the existing contact information to be more specific with "who does what" in each department. How do you know if you were successful? What do your results show? This was successful because we have added the contact list to our website for easy access by internal and external customers. Members of the Administrative Services Work Team can direct customers to the web listing for future reference and the information provided is up-to-date and specific within departments. Additionally, we included an announcement on this update in the first monthly update sent to local districts and all ISD staff. If Goal #1 was not successful, what are your next steps? I believe the goal was successful, but we could increase awareness of the listing by sending out an email notification on the contact us section of the SISD website. Results for Goal #2 Our second improvement goal was to establish a monthly update that is distributed to SISD customers with current, relevant information that may affect them. This goal was established to increase communication with internal (ISD staff) and external (local district) customers. As we at the ISD are informed of important updates and changes, or come across helpful information, we use the monthly update to broadly share that knowledge. How do you know if you were successful? What do your results show? To-date, our group has sent three monthly updates out (January, February, and March 2016). There is now a process in place to ensure that administrative departments, or groups, update their information each month. This has ensured that the update has gone out on time with new information each month. If Goal #2 was not successful, what are your next steps? It would be helpful to have a metric to gauge involvement on the local district and ISD staff end. I recommend, and will bring to the ASWT for discussion, developing a tool for gathering feedback. A simple, one-time could be launched to better understand the impact of the monthly update.

Work Team: Hartley Caretaker Building: Hartley Outdoor Education Center Work Team Leader: Carrie Partlow Results for Goal #1 Being prepared for when groups come out. Being polite and courteous with teachers and students. Communicate with teachers on any needs they may have regarding cleaning, maintenance, or any help needed in general. How do you know if you were successful? What do your results show? Teachers seemed to appreciate the help and were willing to ask if they needed something. Makes their stay more enjoyable and have that at home, everyone is family feeling. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Communicated with staff. Asked for their opinions and recommendations on work performance and any duties they would like to see done. How do you know if you were successful? What do your results show? Worked better as a team. Gave staff the opportunity to see things get done they would like. If Goal #2 was not successful, what are your next steps? N/A

Work Team: REMC and Tech Services Building: RESC Work Team Leader: Matthew Nordquist Results for Goal #1 We’ve outlined areas that needed support beyond one person’s availability and trained one another accordingly. It is an ongoing scheduled process. Some of the areas we have cross trained are as follows: Server - Backup, Recovery, and Troubleshooting Network - Implementing and Troubleshooting User Administration - Creation and Suspension Accounting - Billback and Ordering MDM (Mobile Device Management) Systems - Casper, Lightspeed, Google Device Manager, and Profile Manager Security Camera Systems - ExacqVision, OnSSI, Video Insight How do you know if you were successful? What do your results show? We’ve taken opportunities to practice these tasks as they have come up. We have been successful in this endeavor. Due to this cross training, we have also been able to continue our full support even with experiencing staff turn-over. If Goal #1 was not successful, what are your next steps? Successfully cross trained on areas focused upon throughout the year, however it is an ongoing process for additional areas of expertise. Results for Goal #2 REMC 9 had a goal of surveying its membership and ascertain information to steer professional development activity and increase participation. How do you know if you were successful? What do your results show? The survey results suggested a need for Chromebook and Google training. As a result, REMC 9 offered multiple Chromebook and Google training events and participation increased significantly this year. If Goal #2 was not successful, what are your next steps? N/A

Work Team: School Social Workers Building: RESC/Special Education Work Team Leaders: Jan J. Gaertner and Vickie Dillon Results for Goal #1 We expanded our list of agency visits to include agencies that offer specific services focusing on the needs of ASD students. We also wanted to learn what new programs were available such as the "Saginaw Youth Build" housed at 1000 Tuscola St. in Saginaw, MI. Speaking first hand with their staff allows SSW's to identify the students who could benefit most from such a unique program. How do you know if you were successful? What do your results show? We were successful in obtaining the necessary information to help a student or their family in navigating through such agencies and securing services. Our results proved to be successful as we now know what the application process involves, documentation requirements needed to meet eligibility, as well as making an appropriate referral. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Administration asked our work team to create an evaluation tool to effectively focus on the skills needed in completing the responsibilities of a School Social Worker. The current evaluation tool is very broad and is used for all Ancillary Staff. How do you know if you were successful? What do your results show? We were successful in completing this task, and submitted the evaluation tool to Administration in Nov. 2015. However, Administration stated that they did not have enough time to review the proposed evaluation tool so School Social Workers will continue to use the SIFT Evaluation tool for the 2015/2016 School Year. If Goal #2 was not successful, what are your next steps? N/A

Work Team: PSTP Improvement Team Building: Transitions Center Work Team Leader: Trish Duran Results for Goal #1 To improve students' overall feeling of safety at TC, we incorporated safety training at the start of each semester for each of their classes. This allowed the students to learn where they should go prior to any drills. We then conducted drills throughout the year. At the beginning of the year, we would "warn" students of the upcoming drill to allow time for students and staff to review the procedures. Towards the end of the year, we conducted the drills w/o warning. This allowed everyone to react in a natural situation. After those drills, we allowed time for students to discuss their feelings and time for staff to reward and/or reteach appropriate responses. How do you know if you were successful? What do your results show? We conducted a survey. We polled 74 students (out of 106). In summary, the results showed that 97% of students feel safe while at TC. Approximately 9097% of the students had an opportunity to participate in a drill, wither fire, tornado or critical incident. On average, 90% of the students felt staff were helpful during the drills. While on average, 92% of students knew what to do during the drills. If Goal #1 was not successful, what are your next steps? Overall, the goal was successful. Looking closer at the details, we may need to adjust when we are conducting drills to get more participation. I think increasing students’ knowledge of what to do can also be achieved. Results for Goal #2 Last year, we did not do well on SPP13. We actually did not achieve 100% on any of our selected records. Although most other districts found themselves in the same spot as us, we wanted to strive for 100% compliance. Our teachers participated in 4 trainings throughout the school year. Administrators also provided immediate feedback after each IEP that allowed us to know where we were successful and where, if any, could improve. As a team, we did a file review on almost all of our files in February and did amendments for any IEPs we found out of compliance. This year we had 23 students selected by the State for review. Out of those, only 2 did not meet all of the standards. That's a 87% increase from last year! Our goal is to get to 100% for next year! How do you know if you were successful? What do your results show? Yes and no. Yes, we improved the quality of our IEPs. We conducted all of our IEPs on time. However, we did not achieve 100% as we had hoped. If Goal #2 was not successful, what are your next steps? Our next step is to complete a SLCAP for the State. When we did our file reviews in February, we determined that we need to conduct all new student IEPs prior to March 1st. That way we can ensure it meets all of the SPP13 requirements as many of them did not.

Work Team: Maintenance/Custodial Building: Building & Grounds Work Team Leader: Kelly Quimper Results for Goal #1 We consolidated custodial supplies (cleaning chemicals). How do you know if you were successful? What do your results show? We saved money by using cleaning chemicals that have more than one cleaning use. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Revised the MSDS document system to the OSHA standard hazcom GHS program. How do you know if you were successful? What do your results show? We have complied to the requirements OSHA regulations. If Goal #2 was not successful, what are your next steps? N/A

Work Team: School to Work Building: RESC Work Team Leader: Pat Bray Results for Goal #1 Online template was created and implemented. STW team worked with LEA's to facilitate this process. How do you know if you were successful? What do your results show? Referrals were submitted to Special Education Assistant Director and then were forwarded to STW staff. STW staff met with students in each district accordingly. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 1)A televised informational program was developed with Disability Network. Program was shown live and informed the public about the Saginaw ISD school to work program in districts. 2) Goodwill and MRS established and developed work sites and internships to assist students with disabilities in their vocational goals. 3) STW team continues to participate in OSE trainings to further learn about working with other agencies. How do you know if you were successful? What do your results show? 1) Televised event for work based learning was aired and successful 2) MRS facilitated 8 internships and numerous trainings and job sites through our relationship with Goodwill. Increased MRS successful closures as a result of this relationship. 3) Attended OSE work shops and disseminated information to the STW team. If Goal #2 was not successful, what are your next steps? Work Team: Early Childhood and Parenting Services Building: Cross-ISD Work Team Leader: Noel Kelty Results for Goal #1 Our team collected input from over 584 families and providers regarding program services and needs via survey, utilizing that feedback for program quality, development, and improvement. We utilized the results of the family survey, developed by School Readiness Advisory Committee, to develop a cross-disciplinary professional development event to promote language development between children and caregivers. How do you know if you were successful? What do your results show? Dissemination of the family/caregiver survey was successful, as our committee had over 584 respondents, representing all LEAs within Saginaw County. Parents and caregivers responded that they most wanted strategies and tools to communicate with their child. The School Readiness Committee designed a train-the-trainer workshop intended to provide child care partners, agencies, and schools with tools and a workshop session they could deliver within their own program that would give parents and providers strategies for improving communication for school readiness and social-emotional foundations. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Our team created, delivered, and evaluated a social-emotional early childhood professional development event, based on family survey, for early childhood providers, early elementary principals, and mental health providers. Our team utilized the results of the family survey developed by the School Readiness Advisory Committee to develop a cross-disciplinary professional development event. Planning included representatives from the School Readiness Advisory Committee, and members from the Home Visiting Partners group. How do you know if you were successful? What do your results show? Following the results of the parent survey, it was agreed that the social-emotional topic parents and providers needed the most support on was tools and supports for developing communication skills between caregivers and children to support social-emotional competencies, which lead to a culture of school readiness. The workshop was delivered by Dr. Debbie Lively of Saginaw Valley State University and Christie Alles of the Regional Preschool Partnership on February 18, 2016 and was attended by approximately 39 participants, including early elementary teachers, home-based child care providers, Head Start/Early Head Start Staff, CAN Council Staff, Underground Railroad Staff, GSRP teachers and staff, child care center staff, parents, and others. All participants received a power point presentation to utilize within their own program or service, as well as supplementary materials. Additionally, a program evaluation was completed to determine training success and next steps. Program evaluations were positive, with scores ranging 4-5 on a 5-point scale. Comments included things like the presenters were informative, offered numerous strategies, and included numerous resources. When asked about ideas for future trainings, participants suggested that the next training be about working with strong emotions, temper tantrums, and school readiness. The School Readiness Advisory committee is in planning for a fall 2016 event aligned with survey results. If Goal #2 was not successful, what are your next steps? N/A

Work Team: Kitchen Building: Hartley Outdoor Education Center Work Team Leader: Lisa Slancik Results for Goal #1 We will inquire with our customers throughout their visit to make sure that their needs and wants are met. Our goal is to make their stay here as accommodating as can be. How do you know if you were successful? What do your results show? A. We were very satisfied with the accommodations that were made for all the students and one of our teachers. B. Expectations were absolutely exceeded. The ladies were kind and accommodating. I am not used to dealing w/ such caring and accommodating staff. C. Absolutely! You are so accommodating! No suggestions or ideas. D. Exceptional as always. E. Yes! The food was wonderful! I enjoyed the coffee, hot chocolate throughout the day! F. Always exceeded our expectations! G. Yes, we had a great time. H. Awesome – KIDS HAPPY I. Yes, you always do great! J. Great food!! K. No everything was excellent!! L. Yes you did. Maybe a fruit/vegetable offering. M. We continue to have wonderful experiences at Hartley with both food and programs. N. Yes, food was amazing! N/A Results for Goal #2 We will incorporate a cleaning chart based on daily, weekly, and monthly cleaning as needed . These records are kept in a binder. How do you know if you were successful? What do your results show? We developed a cleaning chart to better manage and record daily, weekly, and monthly cleaning tasks to be more efficient and up to date on these processes. If Goal #2 was not successful, what are your next steps? N/A

Work Team: Speech Pathologists Building: RESC Work Team Leader: Trish Looby Results for Goal #1 The SLP work team developed a survey and distributed it to parents within their assigned buildings. Parents completed the survey immediately following the IEP. Questions included the following requiring a yes or no response: 1. Did you understand the goals? 2. Did you have input into the iep? 3. Were your questions and/or concerns answered? How do you know if you were successful? What do your results show? The survey was completed by parents immediately following an IEP. The work team analyzed the responses based on the questions. The results revealed 100% yes for each questions completed. This means that parents understood the goals, had input into the IEP and their questions and/or concerns were answered. This goal was achieved. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 It was the work team's goals to establish a library of assessment instruments and protocols to be housed a the ISD building within the Special Education Department. This goal was a continuation from last year's goal as it was not achieved. How do you know if you were successful? What do your results show? This goal was not achieved. The SLP work team established a list of tests that should be included in the library at the ISD for all SLPs to access. It was decided the main testing instruments should be housed within the district and lesser used tests be included in the library. The SLP work team voted to not invest in Q-interactive at this time due to the limited testing instruments available on that system. If Goal #2 was not successful, what are your next steps? This goal will be worked on and completed next year.

Work Team: Post Secondary Building: Millet Learning Center Work Team Leader: Olga Kirsanova Results for Goal #1 We decided to increase communication with parents and other stakeholders by developing Facebook classroom pages. How do you know if you were successful? What do your results show? A few teachers developed Facebook classroom pages. The parents really liked this tool by making comments about it to teachers. If Goal #1 was not successful, what are your next steps? Those teachers who created Facebook classroom pages will demonstrate their accomplishments and will help the rest of the teachers to set up their pages, so we can achieve that goal. Results for Goal #2 In order to boost the moral in the team we wanted to schedule and to organize some peer to peer interactions and activities for staff and students. How do you know if you were successful? What do your results show? Post secondary team proposed hosting Cinco de Mayo celebration and inviting high school team. Administration suggested post secondary team hosting event for whole school. It was not enough time to plan a school wide event. If Goal #2 was not successful, what are your next steps? We are still going to organize a completion celebration since our team has a lot of completing students this year. We still will try to address the issue on how to boost the morale and motivation in the team next year. Our team proposed to organize a picnic for post secondary and high school teams to celebrate completion students with referenced socialization and behavior goals (wait turn, eat own food, peers socialize).

Work Team: Office Building: Hartley Outdoor Education Center Work Team Leader: Marty Guzy Results for Goal #1 Put together a VFT form to gather needed information for scheduling, sending out supplies and invoicing. Worked with Naturalists Team on this. How do you know if you were successful? What do your results show? Necessary information for invoicing was easy to obtain as it was on the scheduling form. All invoices done with no problems. Materials sent out in a timely manner (except for one which was delayed in mail). If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Make an Exit Plan for my retirement. Made checklists, comments on each school visiting Hartley so new person knows a little about each of the schools and lists of my job responsibilities. How do you know if you were successful? What do your results show? Made checklists lists for office work, scheduling, etc. I do not know if this will be successful as my replacement has not been hired. I do believe this will be of great help to the new employee. If Goal #2 was not successful, what are your next steps? N/A

Work Team: TCIT Building: Transitions Center Work Team Leader: Brigette Uhrich Results for Goal #1 Held a curriculum night for the first time; created a FB page and Twitter account; frequently provided Superintendent with pictures/summaries of going-ons at TC How do you know if you were successful? What do your results show? The feedback from curriculum was very positive. The amount of expose TC student have received this year has also been positive. If Goal #1 was not successful, what are your next steps? Although we made progress, there are still other things we would like to do, including: 1. Update the webpage more frequently 2. Hold a New Student Orientation/Curriculum Night before school starts (scheduled for Sept 1) 3. Create a monthly newsletter/flyer for worksites to keep them updated and highlight different ones monthly Update FB more regularly and use it as a way to communicate events with students, parents and graduates Results for Goal #2 We varied the drill schedule to involve all students. How do you know if you were successful? What do your results show? Survey results were as follows:PSTP/STEP: Feel safe at school - 97 % Knew what to do during a fire drill - 95% Knew what to do during a tornado drill - 87% Knew what to do during a critical incident drill - 97% The results show we need to do more training/preparation for tornado drills. WBL - Feel safe at TC - 98% Did you participate in a fire drill at TC - 60% (55% knew what to do) Did you participate in a tornado drill at TC - 66% (65% knew what to do) Did you participate in a critical incident drill at TC - 78% (74% knew what to do) The results show we need to schedule more drills when WBL students are in session. All TC Staff Feel safe at TC - 93% Knew what to do for a fire drill - 92% Knew what to do for a tornado drill - 74% Knew what to do for a lockdown drill - 78% The results show that we do not train new staff, especially non-school staff, about drill procedures. If Goal #2 was not successful, what are your next steps? We have room for improvement. We will schedule more trainings with students prior to drills. We will provide new hirers with information regarding drills and safety locations w/i TC.

Work Team: Transportation Improvement Work Team Building: Transportation Work Team Leader: Donna (DJ) Mikolajski Results for Goal #1 Train all employees hired within the last 2 years, in the proper use of bus equipment. Such as: Car seat installment and use, safety vest installment and use, wheelchair use, seatbelt locks, various other equipment use and bus duties. How do you know if you were successful? What do your results show? We have not been successful in putting together the training program necessary to complete our goal. If Goal #1 was not successful, what are your next steps? Our task team will complete what is necessary to work on our goal. Results for Goal #2 All drivers and assistants will be trained in the proper use of the Braun Lift now being installed on our buses. How do you know if you were successful? What do your results show? The task team has been unable to organize and present the training for the use of the Braun Lift. If Goal #2 was not successful, what are your next steps? Our task team will keep working on organizing and presenting this goal to our staff.

Work Team: Naturalists Building: Hartley Outdoor Education Center Work Team Leader: Paul Roose Results for Goal #1 The Hartley Naturalists will do a follow up contact with the schools. How do you know if you were successful? What do your results show? Thirty-three schools came out to Hartley between Sept. and April and Eighteen were contacted about doing a Hartley follow up. Of those eighteen only five were interested in actually doing a follow up. Four follow ups were conducted and it was found that a follow up with just the teacher was more valuable than a follow up with the teacher and students. In a follow up with just the teacher we were able to have an actual conversation about curriculum, their experience, share some data (such as Plickers test results) and anything else they wanted to share. In a follow up which also included the students, it was mostly a fun questioning period about their experience, what they learned, and what they liked best about Hartley. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 The Hartley Naturalists will improve on the consistency and implementation of Pre/Post testing. How do you know if you were successful? What do your results show? The app Plickers was loaded onto the Hartley Ipads and was then used as the method of pre/post testing. Testing started on a regular basis in Jan. and ran through April. At least eleven schools out of twenty schools possible were tested using the Plickers method, which compared to five out of twenty last year during the same period (an increase of 30% more testing). Students improved their scores from the pre test to the post test by an average of 29.3 points. How a school’s particular pre/post data improved was also shared in a follow up. Schools were very interested in our cutting edge pre/post testing method using Plickers, that at least one school download the app right away.

Work Team: Instructional Services Building: Transitions Center Work Team Leader: Rebekah Hornak Results for Goal #1 Two sessions that were in high attendance were provided through an online classroom design using Google Classroom. Rather than charging, the courses were offered for free and 24 SCECHs were provided to those who completed the required course work. How do you know if you were successful? What do your results show? Enrollment for Academic Vocabulary was 4 and Enrollment for Classroom Management was 15. So enrollment was about the same. The participants will be completing a survey on Friday, May 20th. If Goal #1 was not successful, what are your next steps? While I personally prefer face-to-face training, the online training still allows for teachers to learn on their own time. I think a combination of both still needs to be provided so many of the SISD trainings will be of blended format next year. Results for Goal #2 We still need to work on developing a system for collecting quality feedback. I found districts were over completing surveys and not filling them out. I opted to take the approach of working with my principals and curriculum council to develop a plan for 2016-17. How do you know if you were successful? What do your results show? What I found was that there were too many offerings and not enough specific focus for the county. Feedback at the meetings indicated what exactly the districts would like us to focus on next year. If Goal #2 was not successful, what are your next steps? I developed a three tiered system for PD next year in hopes of developing a focus on the county and improving us a team versus spreading out in the region.

Work Team: OT/PT Building: Millet Learning Center Work Team Leader: Bob Hawke Results for Goal #1 The team will reach a level of at least 70% trained in CPR. Many of us work with medically fragile students. A training was set-up to come to the department. How do you know if you were successful? What do your results show? The goal was to have at least 70% of the team trained in CPR. We have 72% trained with current cards. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Our goal was to Inventory our evaluation tools and make a recommendation for what is needed. This is needed due to the change in Fee For Service and how it is more difficult to share stuff. Evaluation tools are a key component of the therapist’s job. How do you know if you were successful? What do your results show? We discussed throughout the year how the new billing format was working in terms of being able to access evaluation tools. The decision was finalized at the last monthly OT meeting for the year. A recommendation concerning what additional evaluation tools would be helpful was discussed at the final meeting for the year. If Goal #2 was not successful, what are your next steps? N/A

Work Team: Bridges Building: RESC Work Team Leader: Jackie Schacher Results for Goal #1 Work team collaborated across disability areas and shared presentation material with each other and Janet Timbs. How do you know if you were successful? What do your results show? Work team created a collaborative powerpoint presentation to provide local districts with a cursory overview of disability specific information. If Goal #1 was not successful, what are your next steps? N/A Results for Goal #2 Work team determined that they needed additional assistance working with Google Docs, Powerpoint, and Disability specific apps. Aaron Schippert conducted a Google Docs training for Bridges staff. Subsequent scheduled trainings were canceled due to weather and scheduling conflicts and were unable to be rescheduled this school year. How do you know if you were successful? What do your results show? Work team utilized powerpoint, google slides, and google docs to complete the presentation. If Goal #2 was not successful, what are your next steps? N/A
