Updating your EFT/ERA – Provider Web Portal Quick Guide We will use the EFT/ERA information from revalidation or enrollment, but if your EFT/ERA information has changed since your enrollment, you’ll need to update.
1. Login to Provider Web Portal 2. Click EFT/ERA Enrollment link
Note: You only need to complete an EFT or ERA Enrollment Application if your EFT/ERA information has changed since enrollment or revalidation.
3. Click EFT Enrollment Application link or ERA Enrollment Application link

4. Click Continue Important: You will get paper checks for two weeks after submitting an EFT change, so make sure your mailing and billing address are up to date before submitting an EFT change.
5. Complete all required fields and click Continue

6. Attach either a voided check or a bank letter Bank letter must be dated within the past 6 months.
Click Continue.

7. Sign the Agreement Type your name into the signature box and click Review.
Print a copy for your records and then click Submit.

Create a password and answer the challenge questions, then press Submit. You will need this password to check the status of your EFT/ERA application.
Record your EFT/ERA Application Tracking Number, you will need it to check the status of your EFT/ERA application. Your new EFT or ERA information should go into effect within two weeks. If you changed your EFT information, you will receive paper checks for the next two weeks.
