Video Camera Circulation Policy

PLEASE READ THIS FORM CAREFULLY BEFORE SIGNING 1. Video Camera checkout is limited to current LWIT staff and students with a current LWIT ID card. 2. No equipment will be checked out to patrons owing money for lost, missing, or damaged equipment or other Library materials. 3. You are financially responsible for all equipment and pieces thereof that you check out. When you sign for equipment, you acknowledge that it is fully functioning, undamaged, and that all the parts are there. 4. Equipment may be checked out for 7 days. Failure to return equipment on time will result in fines. Fines will be assessed at a rate of $1 per day, to a maximum of $30. 5. Video Cameras may not be reserved in advance. 6. Only one Video Camera may be checked out at a time. 7. Do not leave equipment unattended at any time. Equipment must be safeguarded at all times. 8. You are expected to return equipment to a Library staff member at the Circulation Desk in the same condition it was in when you received it. Staff will inspect equipment carefully and call to your attention any suspected damage or missing parts. You must be present when equipment is being checked in. 9. In the event of damaged equipment, library staff will determine if damage is the result of normal wear and tear. Do not attempt repairs, adjustments or alterations of any kind. 10. If a Video Camera is lost or damaged beyond repair, you will be responsible for all replacement fees.

Statement of Understanding of Equipment Use Policy I have read, understand and will comply with all of the Equipment Use Policy. A copy of this policy has been given to me. I understand that I assume complete financial responsibility for the laptop checked out to me.
