Dear Volunteer Candidate, Thank you for your interest in working with Hamilton City Schools’ students. We appreciate your willingness to give of you time, talent and energy to help in our school system. All volunteers and adults working with our students must go through the following process, before working with children. 1.
Please print out this packet and fill out where necessary. (You will not be able to complete this on the computer.) Complete the HCSD Volunteer Packet and submit to the Fine Arts Office at Hamilton High School (except for Page 6--See #2).

Please call the Board of Education at 887-5000 to make an appointment to have your fingerprints taken. When you go for your appointment, please take Page 6 of the packet to the Board of Education Building, 533 Dayton Street. An appointment is required as they may not be able to accommodate walk-ins. There is no cost to the volunteer. Please indicate all areas you may be serving as a volunteer, e.g. HHS Band, HHS Drama, Rhapsody in Blue or Cutting Edge Show Choir, HAMS, HHS Orchestra, etc.

Upon receipt of the background check, your name may be submitted to the Board of Education for approval as a volunteer if required. The Fine Arts teacher will contact you as soon as we receive approval.

The process for approval can take as many as four weeks to complete. Please plan ahead.

Background checks need to be repeated AFTER 5 YEARS. If you have already been volunteering for HCSD and have had your background process completed, you simply need to complete the volunteer packet and indicate when your background check through HCSD expires.

The Volunteer Packet needs to be updated yearly. To alleviate extra paperwork, a volunteer need only complete Page 5 marked “No Changes” if the volunteer packet previously submitted is still current. RESPONSIBILITIES OF TRIP CHAPERONES:
A. The trip leader or teacher is responsible for the preparation and conduct of the trip and is held accountable by the Superintendent for verifying that the purpose of the trip is achieved. B. The trip leader or teacher will provide you with detailed information about the trip. Please obtain answers from the trip leader or teacher, prior to the trip, to any questions you have concerning the purpose for the trip procedures. C. The students on the trip are governed by the District’s Code of Conduct which prohibits any inappropriate behavior, such as discourtesy, fighting, harassment, drug and/or alcohol use, stealing, and the like. Your responsibility is not to invoke discipline on a student, except in cases of imminent threat to that student’s or other people’s safety or well-being, but to report any student behavior problems or any inappropriate conduct on the part of a fellow chaperone or staff member to the trip leader or teacher as soon as possible.

HCSD FINE ARTS VOLUNTEER PACKET-PAGE 2 D. We ask that you model the behaviors expected of students throughout the time on the trip when you are associated with the students. If you have free time away from students, your behavior will be such that it does not create problems for or embarrassment to the trip leader, teacher, or district. We continue to expect that your behavior will comport with the same expectations applicable to when you are associated with the students, including the prohibition against consuming any alcohol and/or tobacco products. Please keep the trip leader or teacher informed of your whereabouts so s/he can contact you in case of emergency.
