Weekly Incident Summary May 2-15, 2016 HALTON HILLS, ON – Monday, May 2, 2016 09:41 Hours – Fire fighters responded to an alarm activation on Mountainview Rd N. On scene staff met with building personnel who advised that the alarm had not activated. The alarm activation was a malfunction between the building and the monitoring company. Building personnel was advised to contact their monitoring company to check the system. 10:11 Hours – Fire fighters responded to the report of a possible structure fire on Normandy Blvd. On scene, no smoke was visible from the street. The homeowner confirmed that all persons were evacuated. Crews entered the home to light smoke conditions. The fire had been extinguished by the homeowner prior to arrival. Crews found a couch smouldering. In order to minimize water damage the couch was removed from the home and completely extinguished. Crews also performed a primary search, checked for further fire extension and ventilated the home. 13:36 Hours – Fire fighters responded to an alarm activation on Foxtail Court. On scene staff met with the site supervisor. Dust from drywall sanding had activated a smoke alarm in the basement. The site supervisor will have each units alarm system taken off line during construction. Tuesday, May 3, 2016 11:36 Hours – Fire fighters responded to an alarm activation on Guelph Street. On scene staff found work crews on the roof, with tar and asphalt. Building personnel advised that a roof hatch was open during the repairs and smoke had entered the building setting off the alarm system. Fire staff met with the Site Foreman and requested that the cover stay on to eliminate any further alarms. 18:45 Hours – Fire fighters responded to the report of a possible structure fire on Kenney St in Acton. On scene fire staff confirmed everyone was out of the home. Crews entered the home and checked the entire home including electrical devices and detectors. The homeowner was advised to keep an eye on things and if they heard or smelt anything to contact the fire department. Thursday, May 5, 2016 08:24 Hours - Fire fighters responded to an alarm activation on Guelph Street. On scene, staff confirmed this was a false alarm. Building personnel were advised to

ensure their alarm companies are notified before any testing is scheduled and to have the system taken off line while work is being completed. 14:23 Hours – Fire fighters responded to a burn complaint on Deer Run Crescent in Acton. No Permit was available and the fire was extinguished. The occupant was advised of the requirement for Burn Permits for any future burns. 14:56 Hours – Fire fighters responded to the report of a possible structure fire on Upper Canada Court. On scene staff found light smoke throughout the home. After further investigation it was determined that the oven had been accidentally turned on self-clean earlier in the morning and burned off the contents of a pan inside. Crews ventilated the home and checked for readings with the departments detectors, with zero readings found. 16:38 Hours – Fire fighters responded to a carbon monoxide (CO) activation on Marilyn Crescent. On scene staff met with the homeowner who advised that their CO detector had been going off intermittently. Zero readings on the detectors were found. It was determined that the homeowner had a faulty unit. A new CO detector was installed as well as a replacement smoke alarm for the upper level of the home. 21:14 Hours – Fire fighters responded to a burn complaint on Coles Court in Acton. On scene staff met with the homeowner who did have a valid burn permit, but was burning grass clippings. The homeowner was advised the burning of grass clippings was not permitted and to not place any further clippings on the pile. Friday, May 6, 2016 11:56 Hours – Fire fighters responded to the report of smoke in the area on Gairey Drive. On scene a small amount of smoke was visible. Staff met with the homeowner who was burning without a valid burn permit. The fire was extinguished. The homeowner was informed not to burn again until a valid burn permit is obtained. 12:30 Hours – Fire fighters responded to a burn complaint on Trafalgar Road, south of 17 Sideroad. On scene staff met with the homeowner who did have a valid burn permit but was not burning within the parameters of the permit. Fire staff extinguished the fire and advised the homeowner not to burn for the remainder of the day and to no longer burn leaves or any other compostable materials. 17:46 Hours – Fire fighters responded to an alarm activation on Queen St E in Acton. On scene staff met with building personnel who advised that the pull station was accidentally pulled by a child. The pull station and the system were reset on arrival and building personnel was advised to silence the system. 21:17 Hours – Fire fighters responded to a two-vehicle motor vehicle collision in the area of Louisa & Guelph Streets. Halton Regional Police and ambulance was on scene upon arrival. All patients were out of their vehicles. Fire staff assisted ambulance with patient care with minor injuries and used absorbant for leaking fluids. 21:54 Hours – Fire fighters responded to the report of a possible structure fire on Steeles Avenue. On scene it was evident that there was a burn pit in the back yard area of this address, with very little flame visible and not near the structure. Staff met

with the tenant who did have a valid burn permit but was not burning within the parameters of the permit. The tenant was advised not to conduct any more burning for the remainder of the evening. Saturday, May7, 2016 07:43 Hours – Fire fighters responded to an ambulance assist on Hyde Park Drive. While enroute, ambulance advised fire was no longer required on scene. Sunday, May 8, 2016 17:44 Hours – Fire fighters responded to the report of a possible structure fire on Church St W in Acton. On scene staff were advised that the fire was in the laundry room. Fire staff entered the home and extinguished the washing machine fire. The washing machine was brought out into the back yard and the home was ventilated. It is suspected that a belt had burnt off. Damage was limited to the washing machine. 21:37 Hours – Fire fighters responded to an alarm activation on Churchill Rd S in Acton. On scene staff met with building personnel who advised that the plumber had set off the alarm. While working on the sprinkler a surge of water set it off. 21:42 Hours – Fire fighters responded to the report of a large bonfire in the area of Trafalgar Road and 10 Sideroad. Upon arrival, the bon fire had spread to a nearby tree and a wooden decorative yard well as observed from Trafalgar Road. The homeowner was trying to extinguish the fire with a garden hose. Fire staff assisted with the extinguishment. It appeared that an old Christmas tree was thrown onto the fire pit which spread the fire. Once the fire was extinguished the homeowners were advised of the burn permit process. Monday, May 9, 2016 10:56 Hours – Fire fighters responded to a single-vehicle motor vehicle collision on Armstrong Avenue. On scene fire staff found a single vehicle roll over with one female patient still in the vehicle. Fire staff removed the complete driver’s side of the vehicle to enable patient access for ambulance. Wednesday, May 11, 2016 03:00 Hours - Fire fighters responded to a carbon monoxide (CO) alarm activation on Marilyn Crescent. On scene staff met with the homeowner who advised that their CO detectors were chirping. Fire staff replaced the batteries and cleared the scene. Thursday, May 12, 2016 02:16 Hours - Fire fighters responded to a carbon monoxide (CO) alarm activation on Newman Place. On scene staff met with the homeowner who advised that their detectors were alarming. Upon entering the home all the detectors had been removed. All were reinstalled but the alarm would not repeat. The homeowner was advised how to inspect and find a faulty detector. 02:47 Hours - Fire fighters responded to a carbon monoxide (CO) alarm activation on North Ridge Crescent. On scene staff entered the home and using detectors found zero readings. Fire staff replaced the detectors and cleared the scene.

09:02 Hours - Fire fighters responded to an alarm activation on Hyde Park Drive. On scene staff were advised that burnt food in the oven had caused the alarm. 21:34 Hours – Fire fighters responded to a public assist in the area of Mountainview Rd and Guelph St. While crews were filling the apparatus they witnessed a fuel spill from a nearby pump. A moderate amount of gasoline was spilled onto the tarmac. Crews applied absorbant. The manager of the gas station shut down the pumps and was advised to have the issue corrected before restoring power to the fuel pumps. Friday, May 13, 2016 08:11 Hours – Fire fighters responded to the report of a motorcycle collision in the area of Maple Ave & Stewart McLaren Road. Ambulance was on scene tending to a single patient and advised fire was not required. 19:33 Hours – Fire fighters responded to an alarm activation on Church St. On scene staff met with building personnel who advised that a smoke machine was in use prior to the fire alarm activation. Crews investigated and found a stairwell smoke alarm activated. The panel was reset. Staff will suspend further use of smoke machine. 20:08 Hours – Fire fighters responded to a burn complaint on Barber Drive. On scene staff met with the homeowner who did have a valid burn permit and was having a small, controlled fire. The homeowner was advised to allow the fire to burn down and not relight this evening. Saturday, May 14, 2016 03:10 Hours – Fire fighters responded to a carbon monoxide (CO) activation on Silver Pond Drive. Upon investigation it was noted that it was the main floor smoke alarm that had activated. The alarm was silent and functioning when crews checked. No levels of CO found in the residence. The homeowner was advised to clean the alarm and replace if they encountered any further issues. 11:44 Hours – Fire fighters responded to a two-vehicle motor vehicle collision in the area of Mountainview Road and Guelph Street. On arrival patients were being attended to by ambulance personnel. Fire staff disabled the battery systems of the vehicles before being cleared by police. 16:54 Hours – Fire fighters responded to the report of a barbecue gas leak on Wildwood Road. On scene fire staff found a natural gas fed barbecue lying on its side with the gas line broken. Crews checked the home for evidence of gas, with no gas detected.
