Weekly Incident Summary June 6-19, 2016 HALTON HILLS, ON – Monday, June 6, 2016 18:27 Hours – Fire fighters responded to the report of hydro wires down in the area of 15 Sideroad and Fourth Line. On scene fire stood by until Hydro arrived. 18:37 Hours – Fire fighters responded to a motor vehicle collision in the area of 27 Sideroad and Winston Churchill Blvd. Caledon Fire was on scene upon arrival and advised that Halton Hills Fire was not required. Tuesday, June 7, 2016 10:52 Hours – Fire fighters responded to the report of a single-vehicle motor vehicle collision with persons trapped, in the area of Guelph St and McFarlane Drive in Norval. On scene fire staff found the vehicle flipped onto its side in the ditch with a single passenger trapped inside. Fire crews stabilized the vehicle and extricated the patient from the vehicle. Halton Regional Police closed Hwy 7 in both directions. Once the patient was extricated she was attended to by ambulance staff and taken to the hospital. 18:16 Hours – Fire fighters responded to a burn complaint on Guelph Street in Acton. No Permit was available and the fire was extinguished. The occupant was advised of the requirement for Burn Permits for any future burns. Wednesday, June 8, 2016 13:15 Hours - Fire fighters responded to the report of a gas leak on Hamilton Crescent. On scene fire staff detected a strong smell of natural gas. Contractors were digging fence posts and struck a buried residential gas line. Union Gas was contacted to attend on scene. Crews notified homeowners in the area and had them shelter in place. Union gas called in a vacuum truck to access the line and clamp the leak. Homes adjacent to the leak were ventilated. 15:50 Hours - Fire fighters responded to the report of a grass/garbage fire in the area of Guelph St and Mountainview Road. On scene, staff found a small pile of brush burning at the rear of the building. Fire staff extinguished the fire and cleared the scene. 19:46 Hours - Fire fighters responded to an ambulance assist on Miller Drive. Ambulance and Halton Regional Police were on scene upon arrival. Using an ATV, fire staff assisted with removing a patient from the trail between Hungry Hollow and Eaton St. The patient was transported by ATV to the ambulance.

Thursday, June 9, 2016 02:09 Hours - Fire fighters responded to a miscellaneous fire on King Street. On scene fire staff found a burnt out sofa on the sidewalk. Crews extinguished the remains and cleared the scene. 14:54 Hours - Fire fighters responded to a single-vehicle motor vehicle collision on Birchway Place in Acton. On scene staff found a vehicle had jumped a curb and struck an A/C unit destroying it. Power to the A/C unit was shut off and the vehicle was checked for issues. Ambulance was treating the patient for minor injuries. 20:45 Hours - Fire fighters responded to a burn complaint on Trafalgar Road. No Permit was available and the fire was extinguished. The occupant was advised of the requirement for Burn Permits for any future burns. Friday, June 10, 2016 03:25 Hours – Fire fighters responded to a public assist on Banting Road. On scene fire staff assisted the homeowner with a smoke alarm beeping. The battery in the upper floor unit was low causing the alarm to chirp. Crews installed a new battery and educated the homeowner on maintenance procedures. Gas detectors were used to check for any carbon monoxide (CO), with zero readings found. 14:11 Hours – Fire fighters responded to a carbon monoxide (CO) activation on Argyll Road. On scene staff entered the home and found zero readings of CO. It was determined that the detector was faulty. The homeowner was advised to purchase a new detector. 21:23 Hours – Fire fighters responded to a burn complaint on Prince Charles Drive. No permit was available and the fire was extinguished. The occupant was advised of the requirement for Burn Permits for any future burns. 21:59 Hours – Fire fighters responded to a burn complaint on James Young Drive. No permit was available and the fire was extinguished. The occupant was advised of the requirement for Burn Permits for any future burns. 22:37 Hours – Fire fighters responded to the report of smoke in the area on 32 Sideroad. On scene fire staff investigated the contents of the fire. The pit contained Styrofoam insulation and green cedar tree branches. No permit was available and the fire was extinguished. The occupant was advised of the requirement and acceptable material for burning for Burn Permits for any future burns. Saturday, June 11, 2016 06:02 Hours – Fire fighters responded to an alarm activation on Barnes Cr. On scene staff met with the homeowner who advised that the alarm was sounding in the basement service room. Crews checked the area and checked the residence with zero readings found. A sump pump water alarm was sounding behind the water heater, with no problems or water in the area. The battery was removed and the homeowners were advised to replace the detector.

06:30 Hours – Fire fighters responded to the report of smoke in the area of Regional Road 25 and St Helena Road Upon further investigation we found the smoke to be coming from a property on Third Line. No burn permit was available and the homeowner was advised to use their on-site tractor to add dirt to smother the fire and to obtain a permit for a future burns. 12:24 Hours – Fire fighters responded to an alarm activation on Lucinda Place. Enroute fire staff were advised this was a confirmed false alarm. On scene staff met with the homeowner who was burning incense. 16:07 Hours – Fire fighters responded to the report of a three-vehicle motor vehicle collision on Main St N. Halton Regional Police and ambulance were on scene with all patients out of their vehicles being assessed by ambulance upon arrival. Fire staff applied absorbant for leaking fluids and disabled two of the vehicles at the request Halton Regional Police. 16:49 Hours – Fire fighters responded to the report of a person trapped in an elevator on Churchill Rd S in Acton. On scene fire staff found a distressed patient stuck in the elevator near the 4th floor. Fire crews locked off the power to the elevator and opened the doors to free the patient. Ambulance assessed the patient, with no medical aid required. Sunday, June 12, 2016 04:04 Hours – Fire fighters responded to an alarm activation on Craig Cres. On scene fire staff entered the home and using gas detectors, found zero readings. 14:33 Hours – Fire fighters responded to the report of a possible grass fire on 17 Sideroad. Upon arrival fir staff found that brush and a fence line were on fire. Crews extinguished the fire along the fence line and traced it back to a pile of brush. The source of ignition was not determined. 16:03 Hours – Fire fighters responded to an alarm activation on John Street. On scene fire staff were directed to an apartment where food was left unattended on the stove. Crews checked around the stove area with no problems found. The apartment was ventilated and cleared the scene. Monday, June 13, 2016 12:40 Hours – Fire fighters responded to the report of a possible grass fire on Regional Road 25. On scene the fire had been extinguished by the homeowner. No Permit was available and the homeowner was advised of the requirement for Burn Permits for any future burns. 18:45 Hours – Fire fighters responded to a burn complaint on Peel Street. On scene fire staff found a small contained controlled fire. The permit had expired and the homeowner was advised to extinguish the fire and reapply for a permit. Tuesday, June 14, 2016 14:24 Hours – Fire fighters responded to an ambulance assist in the area of Trafalgar Road and Highway 7 for the report of a cyclist struck by a vehicle. Halton Regional

Police and ambulance were on scene upon arrival. The cyclist received minor injuries when struck by the mirror of a passing car. 15:29 Hours – Fire fighters responded to a carbon monoxide (CO) activation on Main St S in Acton. On scene fire staff could hear an alarm activating. Fire staff entered the unit and found a plug-in type CO alarm in full alarm. The CO detector was removed from the outlet and put it in to another outlet and the unit stopped. The apartment was being painted and it is believed that the outlet did not have any power which caused the unit to go into alarm. The unit was checked and no trace of CO was detected. 17:34 Hours – Fire fighters responded to a burn complaint on Peel Street in Acton. On scene fire staff found a small contained controlled fire. The homeowner did have a valid burn permit and was burning within the parameters of the permit. 20:42 Hours – Fire fighters responded to the report of a possible structure fire on River Drive. On scene staff found a small fire just inside a window on the west side of the building. Crews extinguished the fire and cleared the scene. Wednesday, June 15, 2016 02:11 Hours - Fire fighters responded to an unknown odour investigation on Guelph Street. On scene staff met with building personnel who advised that cleaners used a floor stripper which caused a couple of employees to not feel well from the fumes of the product. Fire staff checked the interior of the building with monitors and found 0ppm readings and no evidence of any fumes. 12:47 Hours - Fire fighters responded to the report of smoke in the area on Third Line. On scene fire staff found the property owner burning beside a vacant house. The homeowner did have a valid burn permit but was not burning within the parameters of the permit. The fire was extinguished and the permit revoked. 13:20 Hours - Fire fighters responded to the report of wires down in the area of Mary St & Princess Anne Drive. On scene fire staff found a cable wire down. Hydro was notified that they were not required. The cable was moved off the road to allow traffic to pass. 19:21 Hours - Fire fighters responded to an alarm activation on Orville Road in Acton. On scene staff met with the homeowner who advised cooking on the stove had set the alarm off. 20:39 Hours - Fire fighters responded to a burn complaint on River Drive. Halton Regional Police were on scene upon arrival. Light smoke was showing from the southeast corner of the property. Fire staff walked to the southeast corner to investigate. After investigation, the fire was located on the outside of the property near the southeast corner. Crews made their way to the location and met up with Halton Regional Police who were attending to two individuals at the scene. It was a small camp fire and Halton Regional Police had already extinguished it.

Thursday, June 16, 2016 22:22 Hours - Fire fighters responded to a burn complaint on Peel St in Acton. On scene staff met with the homeowner who did have a valid burn permit. Due to the nature of the complaint this evening the homeowner was advised to extinguish the fire. 22:55 Hours - Fire fighters responded to the report of an unknown odour on Donaghedy Drive. On scene fire staff detected the smell of natural gas in the area outside the home. Neighbouring property owners confirmed they had been smelling it for at least a couple of weeks. Fire staff checked the interior of the home and found no readings of gas. Staff was unable to find any source and the smell had dissipated in the area. Union Gas was contacted to respond. Friday, June 17, 2016 05:41 Hours – Fire fighters responded to an ambulance assist on Raylawn Crescent. On scene fire staff assisted with packaging the patient and cleared the scene. 14:59 Hours – Fire fighters responded to a two-vehicle motor vehicle collision in the area of 17 Sideroad, west of Winston Churchill Blvd. On scene fire staff stabilized the vehicles and turned the scene over to Halton Regional Police. 15:42 Hours – Fire fighters responded to an alarm activation on Berton Blvd. On scene the smoke alarm on main floor was sounding. Crews checked the hard-wired smoke alarm and found them dated more than 15 years old. No other problem was found. The homeowner was advised to replace all smoke alarms and the carbon monoxide (CO) alarm in the home. 16:04 Hours – Fire fighters responded to the report of a single-vehicle in the ditch on Guelph Line in Milton. Halton Regional Police were on scene upon arrival. On our arrival our assistance was not required. Cleared the scene. 20:33 Hours – Fire fighters responded to a burn complaint on Danridge Crescent. On scene staff met with the homeowner who did have a valid burn permit. Due to the nature of the complaint this evening the homeowner was advised to extinguish the fire. Saturday, June 18, 2016 14:29 Hours – Fire fighters responded to a two-vehicle motor vehicle collision in the area of Trafalgar Road, north of 17 Sideroad. Halton Regional Police were on scene upon arrival. Fire crews stabilized both vehicles and applied absorbant to leaking fluids on the roadway. Additional crews assisted with traffic control and patient care. Two patients were taken to the hospital by ambulance. 22:18 Hours – Fire fighters responded to a burn complaint on Chetholme Place. On scene staff met with the homeowner who was burning normal combustible materials in a fire bowl, however, without a valid burn permit. The fire was extinguished by the homeowner and the burn permit process was explained. Sunday, June 19, 2016 06:41 Hours – Fire fighters responded to the report of a vehicle into a tree on Delrex Blvd. Halton Regional Police were on scene upon arrival. The vehicle had left the

roadway and struck a fire hydrant. The driver of the vehicle was not found on scene. Crews stabilized the vehicle and cleared the scene. 10:25 Hours – Fire fighters responded to the report of a vehicle in the ditch in the area of Tenth Line and 22 Sideroad. Ambulance was on scene upon arrival. A vehicle was found down an embankment into a group of trees. The occupant of the vehicle was able to free themselves and was being treated in the ambulance. Crews checked the vehicle, which was stabilized in place by a group of trees that prevented the vehicle from rolling and plunging into the pond below. Halton Regional Police arrived on scene and arranged for a tow truck. 18:30 Hours – Fire fighters responded to a carbon monoxide (CO) activation on Russell Street. On scene fire staff checked the home using gas detectors and found no readings. Staff replaced the battery in the detector and the beeping stopped. 20:37 Hours – Fire fighters responded to a burn complaint on Regional Road 25. On scene staff found the homeowner burning green branches in a barrel that spread to a large area of brush, including trees, brush and ground cover. Weekly Safety Message Make a home escape plan.
