Go to BlueCross BlueShield You should see a webpage opened in a new window like the image below.
2. Find Your Doctor Note: If you do not have a primary care doctor, you can skip this section and go to next section. 2.1. Click on "Find a Doctor".

2.2. Find your doctor by entering "zip or city, state" and "provider's name, specialty, facility, or keyword" and then click "Search". The easiest way to find your doctor is to seach by name.
You should see the list of search results like the image below.
2.3. Look though the results to find your doctor. Once you have found your doctor, move your mouse cursor over "Is my plan accepted here?"
You should see a list of plans names appear to the right like the image below. Copy this list down for later use.
Once you have copied down the list of plans, you can close this page and return to BlueCross BlueShield of Arizona webpage.
3. Apply for Subsidy 3.1. Make sure the "Get a Quote" tab is selected.
Click the "Get a Quote" button.
You should see a new webpage like the image below.
3.2. If your income is over the list amount, you are not eligible for government subsidy and you can skip this page by clicking on "Skip this step and view 2014 health plans". Then proceed to Section 4A. Get Covered without Subsidy.
If your income is below the list amount or if your are uncertain whether you are eligible, you should answer the questions and then click the "Submit" button.
Depending on whether you are eligible for subsidy under the Affordable Care Act, you should see one of two webpages like the images below.
If you see the above webpage, processed to step 3.3a.
If you see the above webpage, processed to step 3.3b. 3.3a. You do not qualify for subsidy under the Affordable Care Act becasuse your income is either too high or too low. If your income is below 133% of the federal poverty line, you can receive health care through Arizona's Medicaid Program at little or not cost. You can go to the Medicaid website by clicking on "AHCCCS" or clicking here.
If your income is above 133% of the federal poverty line, then you do not qualify for subsidy under the Affordable Care Act because your income is too high. Click on the arrow on the right and then proceed to Section 4A. Get Covered without Subsidy.
3.3b. Congratulations! You qualify for subsidy under the Affordable Care Act. Click on the arrow on the right and then proceed to Section 4B. Get Covered with Subsidy.
4A. Get Covered without Subsidy 4A.1. You should see a page like the image below. Fill out the form, make sure to add everyone that you plan to cover, then click the "Get My Quote" button.
You should see a list of health insurance options like the image below.
4A.2. If you do not have a primary doctor and skipped the "Find Your Doctor" section of this guide, then you can proceed to the next step. It is time to check this list of health insurance plans against the list of plans accepted by your doctor that you copied down from the "Find Your Doctor" section of this guide.
Note: Of all the plans accepted by you doctor, some of them, such as "Alliance" or "Select", are included in the plan name. Other, such as "PPO" or "EPO", are plan types. You can filter the list of health insurance plans by using the the check boxes on the left. You can also view the plan type and other details by clicking on "Detail".
4A.3. Once you have chosen your plan, it is time to get covered. Click on the "Buy Now" button of your chosen plan.
4A.4. You should see a page like the image below. Verify the plan you chose and click the "Apply Now" button.
4A.5. You should see a page like the image below. Fill out the form and click the "Register" button.
A popup like the image below should appear. Click the "OK" button to countine.
4A.6. You should see a page like the image below. Click on "Start Application".
4A.7. You should see a page like the image below. Fill out the form and then click on the "Continue >>" button at either top or bottom of the page.
4A.8. You should see a page like the image below. Click on the "Continue >>" button at either top or bottom of the page.
4A.9. You should see a page like the image below. Review your information and then click on the "Continue >>" button at either top or bottom of the page.
4A.10. You should see a page like the image below. Take your time to review the acknowledgements. Then electronically sign the application by typing your name and click on the "Submit" button at either top or bottom of the page.
4A.11. Follow the instructions on the page to input your payments information and complete your application. Congratulations you are now covered.
4B. Get Covered with Subsidy
4B.1. You should see a page like the image below. Fill out the form and click the "Register" button.
A popup like the image below should appear. Click the "OK" button to countine.
4B.2. You should have been redirected to the government health care website and see a page like the image below. Please login using an existing account or create a new account. If you are creating a new account, have your email ready for email verification.
4B.3. Once logged in, click on "Apply and shop for coverage for me and/or my family" and you should see a page like the image below. Follow the instructions on the page to verify your identity and your income.
4B.4. Once you have received your subsidy quote, click on "Return to issuer website".
4B.5. You should see a page like the image below. Fill out the form, make sure to add everyone that you plan to cover, then click the "Get My Quote" button.
You should see a list of health insurance options like the image below.
4B.6 If you do not have a primary doctor and skipped the "Find Your Doctor" section of this guide, then you can proceed to the next step. It is time to check this list of health insurance plans against the list of plans accepted by your doctor that you copied down from the "Find Your Doctor" section of this guide.
