Address and dates of all the places you have lived in the last five years Address and dates of all the places you have worked in the last 5 years Your spouse’s name, date of birth, social security number and A# or date of naturalization. (When applicable) Date of marriage with your current spouse, dates of all previous marriages and divorces and copies of all divorce decrees for you and your current spouse Names, dates of birth, country of birth, A#’s (if available) of all your children and current home addresses for all children. Date of arrests, location of arrest, charges, and final dispositions. You must demonstrate that you or someone in your immediate family is receiving a means tested benefit. You will need to provide evidence in the form of an official letter from the agency granting the benefits or other documents that contain:  The name of the agency, name of the person receiving the benefit, type of benefit the person is receiving, and date the person began receiving the benefit. 
