Discussion Items: Item 1: Practice Cohort Application Update: Of the 100 sent letters of acceptance, all were highly recommended candidates. o Selection took multiple other considerations into account such as geography, payer support, and practice type. 33 practices were rural, 33 were pediatric with 14 mixed family care with a pediatrician, and 6 were residency practices. Roughly 64 practices served underserved populations. o Those other highly recommended candidates were placed in a wait-list. A formal list of all the accepted practices may not yet be published yet as the cohort will only be final once all practices return signed agreements. o Deadline set for later in January and reminders will be going out on a regular basis. o Roughly 50 practices have signed by this point. o SIM is working on a dynamic map showing all SIM funded organizations which may help demonstrate the cohort’s reach and locations. Item 2: Multi-Payer Collaborative Update: Patrick Gordon spoke to the current efforts of the Multi-Payer Collaborative: o The MOU is close to final and the last statement of commitment is due Friday. The MPC will be meeting next week to work on the final details of the MOU with an expectation of an agreement by the 20th and a final public MOU by the beginning of February. o The MOU is an exercise in alignment with flexibility for the Payers. It is not binding or uniform, but that it will still be a major undertaking and success in terms of garnering participation in SIM activities.

Meeting Minutes Item 3: Implementation Guide, Milestones, Toolkits, and Training: The Implementation Guide: Will help PTO’s and CHITA’s in coaching and support as well as help demonstrate the concrete steps of practice transformation that practices will follow. It builds off of what was started with CPCI. The Toolkit: a compendium of resources. Versatile tools and a robust set of online resources setup in a way so if for example you were interested in team-based care there would be a set of resources, guides, and tools specifically for team-based care. Practice Transformation Organization Training: All participating PTO’s will be trained on SIM specifics and elements of integration. The training will be January 20-21. Item 4: Quality Assurance Committee: An item for future meetings, it is being brought up now to prompt later discussion. The QAC will manage the efforts of the PTO’s and practices with their progress along transformation. o It will help with the guiding frameworks of the cohort: The Milestones will vary for a number of reasons such as geography, a practice’s needs and strengths, or their level of current integration. While it varies, the milestones provide a degree of objectivity which will allow for assessment and a setting of standards. It will be a learning environment, not a punitive measure. The Committee will balance out an understanding of the challenges inherent to this work while pushing practices to address their weaker areas. The goal is to help identify where PTO’s or practices are having difficulty and help them to overcome issues. A PTO may have practices at varying levels of integration or geography which can be difficult. The QAC can help share best practices and make that variation easier to manage. o The makeup of the Committee will need to be discussed at future meetings and more of this work will need to be further framed out. Item 5: Practice Transformation Funds: The Colorado Health Foundation will give an additional $3 million to the funds earmarked for practices serving predominately Medicare or Medicaid populations. An additional Federal match may also be available, but that is still uncertain. o Additional discussion regarding the uses of the funds by a practice will still need to happen at a future workgroup meeting as more details about what is or is not allowable can be determined from CMS. Workgroup members expressed a need to avoid being overly prescriptive with the funds and to be prepared for unexpected costs and needs. o There will also be low-income loans available for practices to use which would not have restrictions associated with them.

Meeting Minutes Item 6: SIM Office Updates: CMMI Site visit is scheduled for 3 days and will cover every element of SIM. Will be January 19-21. o SIM Operations plan is close to final and will be posted once finished. CDPHE-Local Public Health Agency funding: The 8 LPHA’s have been selected and a press release will be issued shortly. The SIM Internal Evaluator has been selected: Tri-West from Boulder. Still a lot to work out and a contract has not yet been signed. Ashlie Brown from CHI will be running the Colorado Extension Service and the Regional Health Connectors. She will present to the Workgroup in the future on an ongoing basis.
