What specifically, should SIM ask on the next application to the Practice Transformation cohort that would allow us to better catalogue and understand the practice transformation landscape? What other initiatives, outside of SIM, are touching practices? Where are there potential areas of duplication? What is the best forum for convening representatives from various practice transformation efforts to promote alignment?
