Fairfield County School District (FCSD) invites you to submit the name of a person whom you believe is worthy of selection for the Fairfield County School District Hall of Fame. Completion and submission of this form will constitute your nomination of that person for membership in the Hall of Fame. The person you nominate for induction into the Fairfield County School District Hall of Fame should have: shared an inspiring vision for the education of our students; made notable contributions to the educational policies programs; and/or

made a significant contribution to society at large as a result of their education in the District’s schools. Please complete all four (4) sections of this form. Write your nomination with the assumption that the persons who will be reading it will not have any prior knowledge of the person you are nominating for the Hall of Fame. Provide as much specific information as you can about your nominee and his/her achievements and contributions, as well as why you believe he/she should be selected for induction. Applications should be neatly printed or typed in 12-point font size or larger. Attach NO MORE THAN two additional sheets of supporting documents. Be sure to include your name, address, and telephone number should the Selection Committee need to contact you for additional information. Nominators may submit only one (1) nomination per calendar year for consideration by the Fairfield County School District Hall of Fame Selection Committee. Nominations of persons not selected for the induction ceremony in a given year will remain active for two years for future consideration. Nominations for the 2016 induction class are due by Noon, Monday, November 30, 2015. Hall of Fame Overview The Fairfield County School District Hall of Fame Induction program was established in 2013 to honor graduates of Fairfield County School District and other persons whose interests and activities – whether professional, vocational or voluntary – have resulted in significant contributions to the betterment of the school district, the community, state, or nation. The achievements of Fairfield County School District Hall of Fame inductees will have reflected honor on the District. Inductees will be persons whose personal qualities reflect the values of good character that Fairfield County Schools seek to promote in its students: respect, fairness, kindness, cooperation, honesty, dependability, responsibility, good citizenship, caring, and self-discipline.

Eligibility Persons, living or deceased, who have made significant contributions to Fairfield County School District, to their communities, to their professions or to society are eligible for induction into the Fairfield County School District Hall of Fame. Nominees may or may not be alumni of Fairfield County public schools. Persons eligible for nomination for the Hall of Fame may include: 

Former FCSD students – Alumni of FCSD public schools are eligible five (5) years after their graduation.



Former FCSD employees – Former FCSD employees are eligible two (2) years after their employment with the District has ended.



Former FCSD board members – Former FCSD board members are eligible two (2) years after their service on the Board has ended.



Community leaders – Please note that current elected officials (i.e. City Council, County Council members, legislators and other political office-holders) are ineligible; however, former elected officials may be nominated.



Parents of former FCSD students



Any other persons who have made in-kind contributions which qualify for recognition in the Fairfield County School District Hall of Fame.

Judging The Fairfield County School District Hall of Fame Selection Committee, a panel appointed by the School Board of Trustees, will recommend a slate of inductees to the Superintendent of Schools.
