Purpose: It is the purpose of ADHD to coordinate all services possible to ensure optimal compliance with preventive and ongoing health maintenance Policy: It is the policy of ADHD to make all efforts to coordinate services indicated during the visit by scheduling appointments on the same day Procedure: 1. Child Health/Women’s Health Services targeted are WIC, Child Health/Maternity/WPH clinic, Immunizations, CC4C, NB/PP Home visits. 2. Efforts to coordinate all services indicated during the visit are made by scheduling appointments for WIC & Child Health on the same day and time. 3. PCM/ CC4C and /or Nurse providing NB/PP HV works with WIC & Child Health/Maternity by making appointments or contacts as indicated. 4.

Immunizations are reviewed and updated at each clinic visit (including WIC). Immunization only patients will be referred for any identified service needed by that client.

5. Documentation will be made in record to indicate effort of coordination.
